Objective To summarize the experiences of retroperitoneal laparoscopic adrenalectomy.Methods Two hundred and forty-four cases of adrenal tumors (male 124 cases and female 120 cases,range from 16 to 72 year old,110 cases in right side,134 cases in left side) were treated by laparoscopic adrenalectomy in this study.Results Two hundred and forty-two cases were performed successfully,and 2 cases converted to open surgery due to blood lossing.The average operative time was 92 min.The average intraoperative blood loss volume was 58 ml.The average hospital stay was 6.2 d.The maximum tumor diameter was 10.5 cm,the minimal was 1.2 cm,and average was 3.25 cm.All patients were followed-up of 1 to 65 months.Since 2010,no complication occurred and operative time was shorter than before.Conclusions Retroperitoneal laparoscopic adrenalectomy is a safe and effective procedure in treating adrenal tumors.More experience could shorter operative time and decrease the occurrence of complication.%目的 总结后腹腔镜肾上腺切除术的手术治疗经验.方法 本研究244例术前均经超声、CT或CT动脉造影(CTA)等证实为肾上腺占位性病变.男120例,女124例,年龄16～ 72岁.右侧110例,左侧134例.244例患者均接受后腹腔镜肾上腺切除术.结果 2例因出血中转开放手术,其余均在腹腔镜下完成手术.平均手术时间92 min,出血量58 ml,术后住院天数6.2d.肿瘤最大径1.2～10.5 cm,术后无并发症发生.随访1～65个月,无死亡病例.2010年前6例术中出现腹膜损伤,2010年后无手术并发症出现,并且平均手术时间较前明显缩短.结论 后腹腔镜肾上腺切除术是安全、可行的手术方式.随着手术经验的增加,可缩短手术时间并减少并发症的发生.
