A device and a method for generating an electronic case report form, and an electronic case report form service providing system and method using the same are provided to computerize the electronic case report form in an easy mode through various electronic case report forms and representing various information and formats of the electronic case report forms through a single format. An image extractor(110) extracts a case report image form used as background when an electronic case report form is generated from a database according to a user selection. An electronic case report form generator(120) generates a sample of the electronic case report form including data input items by inputting one of components as the item to be applied to the electronic case report form based on the extracted image form. A data transformer(130) transforms the sample of the electronic case report form into a webpage form and stores the webpage form to the database(160). An ODM(Operational Data Model) file generator(140) generates an ODM file based on meta-information of the electronic case report form and stores the ODM file to the database. The meta-information includes the format of contents in the electronic case report form and information for connecting the data and the database.
