右心导管检查诊断肺高血压临床价值

右心导管检查(right heart catheterization,RHC)是一种有创检查,将心导管经周围静脉送入上、下腔静脉、右心房、右心室、肺动脉及其分支,在腔静脉及右侧心腔进行血流动力学、血氧和心排血量测定.右心导管检查在肺高血压患者的诊断及治疗中起着举足轻重的作用,是诊断肺高血压的金标准.文章就RHC在肺高血压诊断及鉴别诊断中的作用,肺高血压患者的临床适应证、具体操作和需要测定参数以及临床误区及其安全情况做简要综述.%Right heart catheterization (RHC) is an invasive test in which the catheter goes into the superior or inferior vena cava,right atrium,right ventricle,pulmonary artery and its branches via peripheral vein for the measurement ofhemodynamics,oxygen saturation and cardiac output in the vena cava and right cardiac cavities.Right heart catheterization plays a decisive role in the diagnosis and treatment of pulmonary hypertension.It is also the gold standard for the diagnosis of pulmonary hypertension.In the current review,we summerizes the significant role of RHC in the diagnosis and differential diagnosis of pulmonary hypertension.The clinical indication,parameters measured in RHC,clinical misunderstanding and safety issues are also included in our review.
