小儿胸外科手术麻醉严重并发症的探讨

@@ 胸外科手术创伤大,出血多,有些肺部手术尚有肺功能障碍及脓性分泌物阻塞呼吸道,尤其是小儿的解剖生理有时存在缺陷,术中呼吸、循环功能常受到严重干扰,甚至引起意外死亡.我院小儿胸外科手术230例中,发生严重并发症4例,占1.74%,兹报道如下.%Both respiratory and circulatory accidents are highly possible during thoracic surgery in infancy and children for its extensive traumatization and massive bleeding.They are also related with pathological,physiological and anatomical anomaly.rnAmong 230 such cases treated in our hospital,2 died of sudden respiratory and cardiac arrest during operations for mediastinal tumours.Autopsies revealed that both cases were myocardial metastasis of lymph sarcoma.Another death was due to bilateral pneumothorax and larynx obstruction after colonic substitution of the esophagus.Still another developed severe acidosis during an pneumonectomy for suppurative condition.But the patient was saved by blood gas monitoring.Causes and prevention of above-mentioned complications are also discussed.
