精索血管高位离断对睾丸血流睾酮分泌及生精功能影响的实验研究

摘要

对幼年大鼠做精索血管离断的实验,待大鼠成年后对其睾丸血流指数、血清睾酮水平和睾丸组织学进行研究.实验结果:睾丸血流指数在高位离断组和正常对照组间无显著性差异(P>0.05),且二组均高于低位离断组(P<0.01);HCG刺激后血清睾酮值高位离断组和正常对照组无显著性差异(P>0.05),二组均高于低位离断组(P<0.01);曲精细管平均直径与睾丸组织学评分,高位离断组与正常对照组无显著性差异,二组均高于低位离断组.结果表明幼年大鼠精索血管高位离断后,通过侧支循环代偿尚能保证成年大鼠睾丸有效血流供给,对内分泌及生精功能无明显影响.从而提示精索血管高位离断有可能作为小儿高位隐睾治疗的一种替代术式.%In this study,th blood vessels of spermatic cord were divided in young rats.The testicular blood flow index in the high division group and the control group was comparable(P<0.05),but significantly impaired in low division group(P<0.01).The postoperative and post-stimulative serum testosterone levels in the high division group and the control group were comparable(P>0.05),and significantly greater than those in the low division group and castrated group(P<0.01).The endocrine level in low division group was comparable with that in castrated group(P>0.05).In the mean tubular diameter and the testicular biopsy scores,there was no significant difference between the high division and the control group(P>0.05),but the results in both groups were significantly higher than those in low division group (P<0.01).These results demonstrated clearly that after the high transection of spermatic vessels the collateral circulation would provide an effective blood flow to the testicules and that there was no deterious effects on testicular endocrine and spermatogenesis in adult rats.This study suggests that the high division of the spermatic vessels may be regarded as an effective operation for correction of the high undescendant testis in pediatric surgery.
