高电压脉冲射频治疗难治性眶下神经痛临床评估

摘要

目的:探讨高电压脉冲射频治疗难治性眶下神经痛的疗效及安全性.方法:40例保守治疗无效的难治性眶下神经痛病人随机分为两组,分别接受标准电压和高电压脉冲射频治疗,比较两组的疗效及副作用.结果:术后1月、3月、6月、1年时高电压组有效率均为90％,显著高于标准电压组有效率.除高电压组有25％、标准电压组有15％的病人治疗后出现轻微短暂(5～40天)患侧眶下神经支配区麻木感外,两组均无严重副作用.结论:对于难治性眶下神经痛病人,高电压脉冲射频是非常有效且安全的治疗方法,可作为保守治疗无效时的治疗选择.%Objective:To investigate effectiveness and safety of high voltage pulsed radiofrequency (PRF) for the refractory neuralgia of the infraorbital nerve.Methods:A total of 40 patients with refractory neuralgia of the infraorbital nerve were randomly divided into two groups and then the patients of two groups undergone the treatment of high voltage PRF or standard voltage PRF respectively. The effectiveness and adverse reactions were compared between the two groups.Results:The response rates in the high voltage group and standard voltage group were 90％ and 60％, respectively, at 1, 3, and 6 months postoperatively (P < 0.05). One year after the surgery, the response rate in the high-voltage group remained 90％, while in the standard voltage group it decreased to 50％ (P< 0.01). So far, no serious adverse reactions were observed, except minor transient numbness in the innervation of ipsilateral infraorbital nerve area in the high voltage group (25％) and in the standard voltage group (15％) after PRF (P> 0.05).Conclusion:High voltage PRF is an effective and safe method and can be used as another advisable choice for refractory infraorbital neuralgia patients who respond poorly to pharmacological treatment.
