上颌窦内提升术中发生窦底黏膜穿孔的上颌磨牙缺失患者同期行种植体植入修复的效果观察

摘要

Objective To investigate the therapeutic effect of simultaneously dental implanting procedures on patients with mucosa perforation in the sinus bottom complicated from intra-maxillary ascension operation for repairing missed maxillary molar based on a clinical trial. Method Included in this trial were 24 cases with mucosa perforation in the sinus bottom complicated from intra-maxillary ascension operation for repairing missed maxillary molar, treated by simultaneously dental implanting procedures in our Hospital from Apr. 2014 to Sep. 2015. They were divided into 2 groups according to the height of bone from the bottom of maxillary sinus to alveolar ridge. When such a height was 4~5 mm, it was ranked to group A and when such a height was 6~8 mm, ranked to group B. In this way, there were 10 in group A and 19 in group B. Following the operation, all patients were followed up for more than 1 year, and during this period, periodically performed X-ray imaging was carried out to evaluate the therapeutic effect on them. Result Among the 24 patients with 29 teeth implanted, 27 plants were successfully repaired except 2 fallen off as seen by the end of following up period, which were in group A and successfully replanted later. All the successfully implanted teeth were shown in good functioning without loosening, with no statistical significance (χ2=2.174, P>005) in the successful rates of implants between the two groups. Conclusion Simultaneously dental implanting procedures can be directly performed for cases with sinus mucosa perforation in the sinus bottom complicated from intra-maxillary ascension operation for repairing missed maxillary molar with higher successful rate. However, it should be kept in mind to strictly grasp the indications for such a procedure.%目的 探讨上颌窦内提升术中发生窦底黏膜穿孔的上颌磨牙缺失患者同期行种植体植入修复的临床治疗效果.方法 2014年4月~2015年9月就诊的上颌磨牙缺失、上颌窦内提升术后出现窦底黏膜穿孔患者24例,均同期行种植体植入修复.根据上颌窦底距牙槽嵴的骨高度,将患者分为甲乙两组.其中上颌窦底距牙槽嵴的骨高度4~5 mm者为甲组,植入种植体10颗;该高度6~8 mm者为乙组,植入种植体19颗.术后随访1年以上,期间定期进行X线片检查,分析其临床疗效.结果 随访结果显示,24例患者29颗种植体中,只有甲组两颗种植体脱落,但在后期再种植成功;其余所有种植体均功能良好,无松动现象,但是两组种植体成功率差异无统计学意义(χ2=2.174,P>005).结论 上颌窦内提升术中发生窦底黏膜穿孔可以直接采取同期种植体植入修复术式,但需要严格把握适应证.
