3~6岁儿童血浆β-胶原特殊序列和 N端中段骨钙素变化规律探讨

         

摘要

Objective To investigate the role of plasma beta⁃crosslaps (β⁃Crosslaps ) and N⁃mid fragment of osteocalcin ( N⁃MID) in healthy 3⁃6⁃year⁃old children. Methods Ninety healthy 3⁃6⁃year⁃old children were chosen with stratified sampling method from Guangzhou City Maternal and Child Health Hospital of Yuexiu District from February to April 2013. Ten of them were disqualified. Both males and females were divided into 2�5-3�5 year group and 4�5 -5�5 year group, with 20 children in each group. The height and weight of these children were recorded, and WAZ and HAZ were counted. The fast venous blood samples were collected. Plasma concentrations of β⁃Crosslaps and N⁃MID were measured using electrochemiluminescence immunoassay (ECLIA). All date were analyzed using a SPSS 11�0 software. The correlation among bone metabolic and biomechanical parametersandage,height,weight,WAZ,andHAZofthechildrenwasanalyzed.Results 1)Theserumβ⁃CrosslapsandN⁃MID showed no significant difference between the gender and age groups ( P>0�05 ) . Mean plasma concentrations of β⁃Crosslaps and N⁃MID in healthy 3⁃6⁃year⁃old children were 1�28 ± 0�31 ng/ml and 64�44 ± 15�47 ng/ml, respectively. 2 ) The association between β⁃Crosslaps level and N⁃MID was positive in 3-4-year⁃old boys and girls (P<0�05), and the r was 0�520 and 0�648, respectively. The association was not significant in 4-6-year⁃old boys and girls (P>0�05). 3) N⁃MID and WAZ was closely associated in 4-6-year⁃old male group (r=0�541, P<0�05). Both β⁃Crosslaps and N⁃MID were not significantly associated with age, height, weight, HAZ, and WAZ (P>0�05) in the rest groups. Conclusion Levels of bone biochemical markers are relatively stable in healthy 3⁃6⁃year⁃old children. Mean plasma concentrations of β⁃Crosslaps and N⁃MID in healthy 3⁃6⁃year⁃old children are 1�28 ± 0�31 ng/ml and 64�44 ± 15�47 ng/ml, respectively. Height, weight, age, HAZ, and WAZ can not reflect the present status of bone metabolism.%目的:探讨3~6岁儿童血浆β-胶原特殊序列(β⁃Crosslaps)和N端中段骨钙素( N⁃MID)的变化规律。方法按分层随机抽样选取广州市越秀区妇幼保健院2013年2月至4月3~6岁健康体检儿童90例,排除10例不合格者,得男性2�5~3�5岁组、4�5~5�5岁组和女性2�5~3�5岁组、4�5~5�5岁组,每组各20例。查询记录各研究对象性别、年龄、身高、体重,计算其年龄别体重 Z 分( WAZ)、年龄别身高 Z 分( HAZ)。采集清晨空腹静脉血,集中检测骨代谢标志物β⁃Crosslaps和N⁃MID的血浆水平,检测方法为电化学发光免疫分析法( ECLIA)。结果用SPSS11�0分析各组β⁃Crosslaps、N⁃MID的变化规律,骨代谢生化指标之间的相关性及其与受试者年龄、身高、体重、WAZ、HAZ之间的相关性。结果①男、女β⁃Crosslaps、N⁃MID水平两年龄组比较之间无显著差别(P>0�05),同年龄组不同性别之间无显著差别(P>0�05),3~6岁儿童血浆β⁃Crosslaps和N⁃MID平均水平分别为(1�28±0�31)ng/ml,(64�44±15�47)ng/ml。②在男、女2�5~3�5岁年龄组中,β⁃Crosslaps与N⁃MID的相关性显著(P<0�05),r分别为0�520、0�648,在男女4�5~5�5岁组β⁃Crosslaps与N⁃MID相关性均不显著(P>0�05)。③男性4�5~5�5岁年龄组血浆N⁃MID与WAZ显著相关(r =0�541, P<0�05),其余各年龄组男、女血浆β⁃Crosslaps、N⁃MID与年龄、身高、体重、WAZ、HAZ相关性不显著(P>0�05)。结论3~6岁儿童骨代谢生化指标水平处于稳定水平,随年龄变化的改变不显著,3~6岁儿童血浆β⁃Crosslaps 和 N⁃MID 平均水平分别为(1�28± 0�31) ng/ml,(64�44± 15�47) ng/ml,且与HAZ、WAZ无显著相关性,年龄、身高、体重、WAZ、HAZ不能代表当前骨代谢状态。
