目的 观察和比较不同清洗方法对疑似"尿路感染"女性患者的尿常规化验结果的影响.方法 选择经尿常规化验拟诊为"尿路感染"的女性患者34例,随机分为会阴冲洗组(n=8)、自行家中清洗组(n=12)和医院清洗组(n=14),分别用不同方法清洗会阴,清洗后重新留取中段尿复查尿常规.对清洗前后尿常规结果进行比较,复查尿常规结果仍异常者,再行清洁中段尿培养进行确诊.结果 经3种方法清洗后,82.4%(28/34)患者复查尿常规转阴;6例尿常规仍为阳性,经尿培养确诊3例尿路感染,清洗前后尿常规的假阳性率分别为91.2%(31/34)和50.0%(3/6),差异有统计学意义(x2=6.782,P＜0.05).3组复查尿常规的转阴率分别为87.5%,83.3%和78.6%,3组之间差异无统计学意义(P＞0.05).结论 会阴清洗能够降低尿常规化验的假阳性率.%Objective To observe and compare three different perineal cleaning methods on urine routine test results of women whose preoperative diagnosis is "urinary tract infection".Methods 34 female cases whose preoperative diagnosis is urinary tract infection in our hospital from April 2010 to July 2010 were randomly divided into three groups:perineal irrigation group, self-perineal cleaning in home group and self-perineal cleaning in hospital group.The urine routine test before and after perineal cleaning was check-up and compared.The urine culture was carried in the cases which were still abnormal urine after perinea cleaning.Results After perinealcleaning, 82.4% (28/34) cases have negative urine, 17.6% (6/34) remains positive.The urine culture confirmed "urinary tract infection" in 3 of 6 cases with positive urine; the false positive rate has significant difference (x2 =6.782,P＜0.05) between before (91.2%, 31/34) and after perineal cleaning (50.0%, 3/6).After perineal cleaning, the negative conversion rate of urine routine test is 87.5%, 83.3% and 78.6% in three groups respectively.There are no significant differences between three groups(P＞0.05).Conclusions Perineal cleaning can significantly reduce the false positive rate of urine routine test in female patients.Three different methods, including perineal irrigation, self-cleaning in home and self-cleaning in hospital, can significantly reduce the contamination of urine samples.
