Objective To study the effects of community nursing intervention on the hypertensive patients.Methods Total of 216 hypertensive patients from four villages in Jiuduhe town were selected.They were followed up regularly and popularized with hypertension knowledge and their health records received standard management.One year later,the intervention effects indexes such as the awareness rate of hypertension knowledge,the change of lifestyle before and after intervention,the rate of taking medicine regularly,and the rates of blood pressure control and review,were observed.Results After intervention,the awareness rate of hypertension diagnosis standard and needing medication adherence was 54.63％ and 86.11％ respectively,which was higher than 25.93％ and 49.54％ before intervention,and the differences were statistically significant ( x2 =36.99,66.20 ; P ＜ 0.01 ).Before and after intervention,patients' change of weight control,salt and dink limit,moderate exercise,smoking was significantly different ( P ＜ 0.01 ).The rate of total review,taking medicine regularly and blood pressure control respectively increased by 29.16％,10.65％,11.11％,when compared with that before intervention ( x2 =39.93,4.91,7.02; P ＜ 0.01 ).Conclusions The intervention of building files,standard management and regular community nursing can effectively control hypertension and improve hypertensive patients' life quality,which is good for patients to receive constant and system therapy.%目的 探讨对高血压患者进行社区护理干预的效果.方法 采用简单随机抽样法选择北京九渡河镇4个村的216例高血压患者作为干预对象.为216例高血压患者建立健康档案进行规范化管理,并定期随访,向其普及高血压相关知识.干预1年后评价其干预效果,包括高血压患者相关知识知晓率、干预前后生活习惯的改变、干预前后患者血压复查率、规律服药率及血压控制率.结果 干预后216例患者对高血压诊断标准(54.63％)、需坚持服药(86.11％)等相关知识知晓率均高于干预前(25.93％ 、49.54％),差异均有统计学意义(x2分别为36.99,66.20;P均＜0.01).干预后,患者在控制体质量、限制食盐、适量运动、限制饮酒、戒烟方面均有较大改变,干预前后差异有统计学意义(P＜0.01).干预后高血压患者总复查率、规律服药率和总控制率较于预前分别提高了29.16％,10.65％和11.11％,差异均有统计学意义(x2分别为39.93,4.91,7.02；P均＜0.05).结论 对高血压患者建档实施规范化管理,定期进行社区护理干预,有助于高血压患者进行连续性、系统性治疗,是有效控制高血压、提高离血压患者生活质量的方法.
