Objective To investigate clinical effect by olanzapine or quetiapine combined with donepezil in the treatment of senile behavioral and psychological symptoms of dementia (BPSD).Methods A total of 84 senile dementia patients were randomly divided into olanzapine group and quetiapine group, with 42 cases in each group. Clinical effects and adverse reactions were compared between the two groups.Results Total effective rate was 90.48% in the olanzapine group and 88.10% in the quetiapine group. Incidence of adverse reactions was 11.90% in the olanzapine group and 14.29% in the quetiapine group. Their differences all had no statistical significance (P>0.05).Conclusion Combination of olanzapine or quetiapine and donepezil in the treatment of senile BPSD can effectively accelerate improvement in clinical symptoms. It is safe and creditable, along with good tolerance during treatment. This method can be suggested in further clinical promotion.%目的：探讨奥氮平或喹硫平联合多奈哌齐在老年痴呆精神行为症状(BPSD)治疗方面的临床效果。方法84例老年痴呆患者，随机分为奥氮平组和喹硫平组，各42例。比较两组临床治疗效果及不良反应发生情况。结果奥氮平组总有效率为90.48%、喹硫平组为88.10%，奥氮平组不良反应发生率为11.90%、喹硫平组为14.29%，比较差异均无统计学意义(P>0.05)。结论奥氮平或喹硫平联合多奈哌齐均可有效促进老年BPSD患者临床症状的改善，安全可靠，患者在治疗期间具有较好的耐受性，建议在临床进一步推广。
