不同敷料组合对烧伤患者供皮区创面微环境的影响

         

摘要

Objective To discuss the impact of different dressing combinations of donor site wounds in burn patients microenvironment. MethOd 372 cases involving skin grafts in burn patients for the study , all set in skin thickness 0. 3mm. Based on 400 randomly form donor site wounds do group divided : the first group , alginate pad group ;second group , vaseline pad group;third group , alginate foam group;fourth group , vaseline foam group . The a-bove observation of patients in each group donor site conditions , including pH value of water evaporation and what the impact will be analyzed bacterial colonization and clear the dressing process of patient pain levels, the actual wound healing time. Result The total of 396 patients for skin wounds 368 cases of full participation in the experi-ment, four kinds of dressing combination can create a distinctive microenvironment. The first group to be detected on the surface of the fourth set dressing water evaporation ,the following values were obtained (36. 6±4. 3)ml/(h·m2), (32.4±3.9)ml/(h·m2),(24.2±3.0)ml/(h·m2),(19.2±3.4)ml/(h·m2),with moisturizing properties is concerned, the fourth group of the obvious advantages;pH value on the wound to be detected ,the following values were obtained(7. 33±0. 07),(7. 52±0. 04),(7. 16±0. 04),(7. 45±0. 07);positive rate of bacterial culture on the wound be analyzed found that the third group(4. 0%) the lowest , and the fourth group (22. 4%) the highest;re-move the dressing process for patients with pain level statistics found that the third group of the lightest (0. 99± 0. 13),the second most important group (8. 15±0. 93);statistics to be found on wound healing time , and the third group of the shortest (6. 8±0. 9)d, the longest fourth group (15. 7±3. 6) d. COnclusiOn For the same wound dressings if there are differences ,then create their own micro-environment of the wound will be some differences;healing the environment and the actual elapsed time intrinsically linked;compared to pH , the humidity will be on the wound more direct healing effect more significant impact.%目的:讨论不同敷料组合对烧伤患者供皮区创面微环境的影响。方法选取涉及植皮的372例烧伤患者为研究对象,皮厚度全部设定在0.3mm。基于表格随机法对400个供皮区创面做组别划分:第一组,藻酸盐棉垫组;第二组,凡士林棉垫组;第三组,藻酸盐泡沫组;第四组,凡士林泡沫组。观察上述各组患者供皮区情况,主要包括水分蒸发量以及pH的影响,分析细菌定植信息、清除敷料过程中患者痛感等级、创面实际愈合时间等。结果共计368例患者的396个供皮创面全程参与实验,4种敷料组合能够营造各具特色的微环境。检测第一组至第四组敷料表面水分蒸发量,数值分别为(36.6±4.3)ml/(h·m2)、(32.4±3.9)ml/(h·m2)、(24.2±3.0)ml/(h·m2)、(19.2±3.4)ml/(h·m2),第四组差异显著;四组创面pH分别为(7.33±0.07)、(7.52±0.04)、(7.16±0.04)、(7.45±0.07);四组创面细菌培养的阳性率,第三组(4.0%)最低,第四组(22.4%)最高;清除敷料过程中患者痛感等级进行统计发现,第三组最轻(0.99±0.13),第二组最重(8.15±0.93);四组创面愈合时间,第三组最短(6.8±0.9)天,第四组最长(15.7±3.6)天。结论对于同样创面,如果敷料存在差异,那么各自营造出的创面微环境也将存在一定的差异;环境和创面愈合实际耗用时间存在内在联系;相较pH而言,湿度将会对创面愈合效果产生更直接且更重要的影响。
