正念训练改善更年期妇女幸福感及抑郁症状的随机对照研究

         

摘要

Objective To evaluate the effectiveness of mindfulness training applied in improving menopausal depres-sion and well-being,and to explore new methods in menopausal mental health. Method Premenopausal and postm-enopausal women aged 40 to 60 were randomized to a mindfulness training group(n=29) or to a control group(n=31). Main outcome was assessed with a Self-Rating Depression Scale ( SDS) and General Well-Being Schedule ( GWB) before and after( at the ninth week) the intervention respectively. Result The demographic characteristics and the baseline levels of SDS index and the GWB total score were comparable between mindfulness training group and control group. The SDS index in mindfulness training group was significantly reduced by 0. 046(P<0. 05),and the GWB total score was significantly increased by 5. 6(P<0. 05);in the subgroup of“SDS index≥0. 50”,the SDS index in mindfulness training group was significantly reduced by 0. 095(P<0. 05),and the GWB total score was sig-nificantly increased by 11. 4(P<0. 05). While there were no statistically significant changes observed in SDS index and the GWB total score in the control group. Furthermore,the improvement of the SDS index and the GWB total score in the subgroup of“index≥0. 50” was much sharper than control group. Conclusion Mindfulness training has certain effectiveness in improving menopausal depression and well-being,especially the women who have severer de-pression symptoms.%目的评价正念训练对提高更年期妇女幸福感水平及改善抑郁症状的干预效果,探索更年期心理保健新方法。方法将处于围绝经期的40~60岁妇女随机分为试验组( n=29)和对照组( n=31),试验组接受8周正念训练,而对照组在此期间不接受该训练方法。采用自评抑郁量表( self鄄rating depression scale,SDS)及总体幸福感量表( general well鄄being schedule,GWB)分别评价研究对象的更年期抑郁症状严重程度及幸福感水平,入组时进行基线测评(干预前测评),入组后第9周进行干预后测评。结果两组研究对象的人口学特征、基线测评结果都均衡可比。干预后测评结果显示,试验组的抑郁严重指数较基线测评值降低了0.046,总体幸福感量表总分较基线测评值升高了5.6;在“抑郁严重指数≥0.50冶者中,试验组的抑郁严重指数较基线测评值降低了0.095,总体幸福感量表总分较基线测评值升高了11.4,以上前后差异均有显著性(P<0.05)。而对照组的抑郁严重指数及总体幸福感量表总分在干预前后的差异均无显著性。此外,“抑郁严重指数≥0.50冶者的抑郁严重指数及总体幸福感量表总分在干预前后的变化分别大于总人群。结论正念训练能降低更年期妇女抑郁情绪,提高幸福感水平,特别是对有更年期抑郁症状严重者的改善效果更加明显。
