多项检验指标在恶性疟患者诊治中的临床意义

摘要

Objective To explore the characteristics and clinical applications of clinical biochemical profiles in patients with falciparum malaria.Methods We enrolled 60 patients with falciparum malaria as study group and 60 health people as control group in this study,and compared the difference of biochemical profiles in these two groups.We did a statistical comparative analysis on 60 patients who got falciparum malaria,simultaneously on their pre and afterpharmacotherapy with the parameters of CRP and P-ALB.Results TBil,DBil,AST,ALT,CRP and IL-6 levels were significantly higher in malaria group than those in control group (P＜0.05),and TP,ALB,P-ALB,CHE,TCHO,HDL-L and LDL-L levels were obviously lower in malaria group than those in control group (P＜0.05).In addition,with the treatment course prolonged,The platelet (PLT) levels in falciparum malaria tissue were significantly lower than that in the control group (P＜0.05),and the lowest level was 11.4 × 109/L.The CRP level in the falciparum malaria increased with the treatment time,and decreased with the malaria parasite disappeared.The P-ALB increased with the treatment and conditions were improved.The positive rates of both mycoplasma pneumonia IgG antibody and IL-6 were 87.5％.Conclusion Serum biochemical profiles have importantly clinical value for the diagnosis and evaluation of treatment efficacy in patients with falciparum malaria.%目的 探讨恶性疟患者多项临床检验指标的变化特点及其应用价值.方法 选取2010年1月至2015年7月在本院就诊的60例恶性疟疾患者作为研究组,以60例健康体检者作为对照组,比较两组的多项临床检验指标.同时对60例恶性疟患者抗疟治疗前后血清C-反应蛋白(CRP)和前白蛋白(P-ALB)的水平进行对比分析.结果 研究组治疗前血清总胆红素(TBil)、直接胆红素(DBil)、天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、CRP、白介素6(IL-6)水平均明显高于对照组(P＜0.05),总蛋白(TP)、白蛋白(ALB)、P-ALB、胆碱酯酶(CHE)、胆固醇(TCHO)、高密度脂蛋白(HDL-L)、低密度脂蛋白(LDL-L)水平及血小板(PLT)水平均明显低于对照组(P均＜0.05).随治疗时间延长,研究组CRP水平逐渐降低,P-ALB水平逐渐升高.肺炎支原体IgG抗体阳性率达87.5％.结论 临床血清检验指标对恶性疟患者疾病诊断和疗效评估具有重要价值.
