Objective To investigate the ultrasonographic appearances of various pathologic cystic-solid lesions. Methods The sonographic features pathologically proven breast benign or malignant cystic-solid lesions in 52 cases were analyzed retrospectively. The accuracy of ultrasound diagnosis of breast cystic-solid lesions was evaluated with the area under curves of ROC. Results The sonographic appearances of the lesions were as follows: (1) Predominantly cystic mass with solid components (n=15)which appeared as cystic masses with regular or irregular thick solid margin or local margin protruding into the cystic components; (2) Predominantly solid mass with cystic components (27), including solid masses with cystic regions, or solid masses with surrounding dark regions; (3) Mixed solid-cystic mass (10), in which the solid component equaled to the cystic. Benign lesions (40) in the 52 solid-cystic lesions were more than that of the malignant (12). The begnin lesions could be visualized in all above mentioned sonographic types, whereas malignant lesions were predominantly solid with cystic foci. The area under curves of ROC was 0.893, which was significant for the diagnosis of breast cystic-solid lesions, P<0.01. Conclusions The sonographic features of breast solid-cystic lessions have some characteristics. Recongnition and correct interpretation of them is helpful for diagnonsis.%目的 探讨不同病理类型乳腺囊实性病灶的超声声像图特点.方法 根据病灶中囊性与实性所占的比例,对52例经病理确诊为乳腺良性或恶性囊实性病变患者的声像图表现进行回顾性分析,并以曲线下面积(ROC)评价超声对乳腺囊实性病变诊断的准确性.结果 52例乳腺囊实性病变中超声显示以囊性为主15例,有规则或不规则实性厚壁或囊壁局部隆起,呈乳头样突入囊腔内;以实性为主27例,实性病灶内少许囊性区域或实性病灶周围液性无回声带环绕;囊实混合性10例,病灶内囊性及实性成分大致相等,相互混杂.超声及病理检查结果显示,52例中良性(40/52,76.9%)多于恶性(12/52,23.1%),良性与恶性病变检出率比较差异有非常显著性(P＜0.01).良性病变可见上述各类声像图表现,但恶性囊实性病变主要表现为实性为主内含少许囊性区域.超声诊断乳腺囊性病变的ROC曲线下面积为0.893,对乳腺囊实性病变有诊断意义(P<0.001).结论 乳腺囊实性病变的声像图有多种表现,正确认识不同表现的囊实性病变,有助于提高超声诊断符合率.
