双向内膜下成形术在股浅动脉长段闭塞介入治疗中的应用

摘要

Purpose To investigate the clinical value of interventional therapy for long-segment total occlusion of superficial femoral artery by bidirectional subintimal anginplasty (SIA) technique. Materials and Methods Eleven patients with longsegment total occlusion of superficial femoral artery were treated by using femoropopliteal artery bidirectional SIA technique. The ankle / braehial index(ABI)ofpost-treatment was analyzed and the follow-up time was 12 months. Results 11 cases with long-segment occlusion of superficial femoral artery were recallalized, with technical success rate of 100％. The ischemic symptoms of lower limb improved. The ABI was 0.74±0.26 for post-treatment and 0.34 ± 0.13 for pre-treatment respectively(t=3.35,P＜0.05 ). the patency rate of 3,6 and 12 months after treatment was 100％、 96.9 ％ and 72.7％ respectively.Conclusion The success rate of application of femoropopliteal nrtery dual input subintimal nngioplasty (SIA) technique to treat long-segment total occlusion of superficial femoral artery is high,with accurate short-term effect, it is a new idea for interventional treatment of lower extremity arterial occlusion.%目的 探讨双向内膜下成形术(SIA)在股浅动脉长段闭塞介入治疗中的临床价值.资料与方法 采用股胭动脉双向内膜成形术治疗11例长段股浅动脉闭塞患者,术后复查踝肱指数并随访12个月.结果 11例患者股浅动脉闭塞段均开通,技术成功率100%,下肢缺血症状改善,复查踝肱指数0.74±0.26,与术前0.34±0.13比较明显升高(t=3.35,P＜0.05);术后3个月、6个月和12个月支架通畅率分别为100%、90.9%和72.7%.结论 股胭动脉双向内膜下成形术治疗长段股浅动脉闭塞成功率高,近期疗效确切,为介入治疗复杂下肢动脉硬化闭塞提供了新思路.
