应用三维室壁运动追踪技术探讨正常心肌三维周向应变值的初步研究

         

摘要

Purpose To investigate the normal circumferential strain value in Chinese people by 3DT technique. Materials and Methods Forty-five male and 45 female healthy volunteers of Hah nationality from Beijing, China, aged from 20 to 52 (30.7 in average) years old were recruited in this study. 4D transducer of Toshiba SSH880H ultrasonic diagnosis device was used to obtain the full volume images on apical 4-chamber views. Each image was divided into 17 segments and then the peak 3D circumferential strain value of each segment was evaluated using 3DT technique. SPSS 11.5 software was used to analyze the data. Results The mean value of normal peak 3D circumferential strain of each segment was as following: BA 2. 62 ±5.06, BAS 3. 92±6.49, BS 3. 57±6.74, BI l.24±3.59, BP 1. 94±4.40, BL 2. 50±5.12, MA 0. 73±2.29, MAS 1. 49±2.76, MS 2. 41±4.35, MI 1. 58±4.21, MP 1.87±3.78,ML 1.41±3.59, AS0.59±1.65, AI 1.17±2. 55, AP 1.66±3.57, AL 1.29±3.15, A 1.06±2. 40. The 3D circumferential strain increaseed from apical to basal segments. Conclusions This is the first time to obtain normal 3D circumferential strain value in Chinese healthy people and may provide useful reference in extending the use of 3DT.%目的:应用三维室壁运动追踪(3DT)技术分析正常心肌三维周向应变(3D circum strain)值的分布情况.材料和方法:选择健康北京汉族志愿者90例(年龄20~52岁,平均30.7岁),男、女各45例,所有受试者操作步骤如下:使用常规探头采集胸骨旁左室长轴、短轴及心尖四腔心切面等常规心脏检查切面的图像.在心尖四腔心切面上,采用容积探头采集心脏的全容积图像,并存储原始数据.脱机提取全容积图像原始数据并对图像进行17节段划分,应用3DT技术进行收缩期3D Circum Strain值和达峰时间的计算.所采集数据应用SPSS 11.5软件进行分析.结果:左心室17个节段的3D Circum Strain峰值分别为:前壁基底段(BA)2.62±5.06;前间隔基底段(BAS)3.92±6.49;室间隔基底段(BS)3.57±6.74;下壁基底段(BI)1.24±3.59;后壁基底段(BP)1.94±4.40;侧壁基底段(BL)2.50±5.12;前壁中段(MA)0.73±2.29;前间隔中段(MAS)1.49±2.76;室间隔中段(MS)2.41±4.35;下壁中段(MI)1.58±4.21;后壁中段(MP)1.87±3.78;侧壁中段(ML)1.41±3.59;室间隔心尖段(AS)0.59±1.65;下壁心尖段(AI)1.17±2.55;后壁心尖段(AP)1.66±3.57;侧壁心尖段(AL)1.29±3.15;心尖部(A)1.06±2.40.3D Circum Strain峰值由心尖部到心底部呈现阶梯性分布,以心底部为高值.心尖和中间段达峰时间早于基底段.结论:这是应用灰阶3DT技术对部分国人进行的心肌正常3D Circum Strain的初步分析结果,可以为以后3DT技术的临床应用提供有价值的参考标准.
