新疆地区维吾尔族非酒精性脂肪肝患者血清脂联素水平及与肝脏酶学指标、血脂的相关性

摘要

Objective To investigate the association of the level of serum adiponectin and nonalcoholic fatty liver disease ( NAFLD) in Uyghur ethnic groups .Methods 558 cases were divided into NAFLD and healthy control groups .The general situations of the two groups were measured,including age,weight,body weight,BMI,waist circumference,hip circumference,waist-depth ratio,SBP,DBP,FPG,lipid and UA levels.Results Age,body weight,waist-depth ratio,BMI,SBP,DBP of case group were significantly higher than those of control group . The levels of TG,LDL-C,TC,FPG and uric acid of case group were higher than those of control group ,the levels of HDL-C,adiponectin of case group were lower than those of control group ( P<0.05).Adiponectin was negatively correlated with BMI ,ALT,TC,TG,UA,LDL-C, SBP,DBP,AST,ALT,waist circumference and positively correlated with HDL-C.ALT and BMI were the main risk factors for the formation of NAFLD,and ADP was protective factor for NAFLD .Conclusions The increasing of serum adiponectin level is of great significance to patho-genesis of NAFLD in Uyghur ethnic groups ,and might be a protective factor in the pathogenesis of NAFLD .%目的：探讨新疆地区维吾尔族成人非酒精性脂肪肝（ NAFLD）患者血清脂联素（ APN）水平的变化特点及与肝脏酶学指标、血脂、尿酸等指标的相关性。方法选取558名健康体检者，根据B超结果分为NAFLD组289例与健康照组（非NAFLD组，269例），详细测量并记录两组年龄、身高、体重、体质指数（BMI）、腹围（WC）、腰高比（WHtR）、收缩压（SBP）、舒张压（DBP），测定肝脏酶学指标、空腹血糖（FPG）、血脂、尿酸（UA），同时应用酶联免疫吸附法测定血清中APN等指标。采用非条件 Logistic 回归分析 NAFLD 发生的危险因素。结果 NAFLD 患者 BMI、WHtR、SBP、DBP、总胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白胆固醇（LDL－C）、谷草转氨基转移酶（AST）、丙氨酸氨基转移酶（ALT）、FPG、UA水平均高于对照组，而高密度脂蛋白胆固醇（HDL－C）、APN水平显著低于对照组（P＜0．05）。 Pearson相关分析结果显示，APN 与 BMI、TC、TG、UA、LDL－C、SBP、DBP、AST、ALT、腹围呈负相关，而与HDL－C呈正相关。非条件Logistic回归分析显示，ALT（OR＝1．058，95％CI 1．008～1．109）与BMI（OR＝14．177，95％CI 1．132～1．528）是NAFLD形成的主要危险因素，而血APN水平（OR＝0．981，95％CI 0．975～0．987）是NAFLD发生的保护性因素。结论新疆地区维吾尔族血清APN水平降低在NAFLD发病中具有重要意义，其可能在NAFLD发病过程中起保护作用。
