帕金森病患者血清血管紧张素Ⅱ水平与抑郁和焦虑程度的相关性研究

摘要

目的 探讨帕金森病(PD)患者血清血管紧张素Ⅱ水平(ANG Ⅱ)与抑郁和焦虑程度的相关性及其临床意义. 方法 选取2014年1月至2016年12月在我院神经内科诊治的PD患者60例,并选取同时期健康体检者60例.对研究对象的抑郁和焦虑程度通过汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)进行评估,同时测定研究对象血清ANG Ⅱ水平并分析二者之间的相关性. 结果 PD组抑郁程度和焦虑程度均高于健康对照组[HAMD评分:(23.0±5.9)分比(4.7±2.2)分,t=22.23,P<0.01;HAMA评分:(22.3±9.7)分比(5.2±3.3)分,t=11.71,P<0.0001],而PD组血清ANG Ⅱ水平则低于对照组[343.4(230.0,470.2)ng/L比793.5(440.9,2 087.1)ng/L,U=701.5,P<0.01].Spearman秩相关分析显示PD组患者血清ANGⅡ水平与患者抑郁和焦虑程度均呈负相关(HAMD:r=-0.5468,P<0.01;HAMA:r=-0.4146,P=0.001). 结论 PD患者血清ANG Ⅱ水平与抑郁和焦虑程度显著相关,可能成为预测和诊断PD患者抑郁和焦虑的新指标.%Objective To investigate the correlation of serum angiotensin Ⅱ level with the severity of depression and anxiety in patients with Parkinson's disease (PD) and its clinical significance.Methods Sixty patients with PD at the Department of Neurology in our hospital and 60 healthy individuals who underwent physical examination were included from January 2014 to December 2016.Depression and anxiety were evaluated by Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA),respectively.The serum ANG Ⅱ level was also assayed and its association with depression and anxiety in PD patients was examined.Results Depression and anxiety in the PD group were more severe than in the healthy group (HAMD score:23.0±5.9 vs.4.7±2.2,t=22.23,P<0.0001;HAMA score:22.3±9.7 vs.5.2±3.3,t=11.71,P<0.01),while serum ANG Ⅱ level of PD patients was significantly lower than in the healthy individuals [343.4 (230.0,470.2) ng/L vs.793.5 (440.9,2 087.1) ng/L,U =701.5,P < 0.01].Spearman's rank correlation analysis suggested that serum ANG Ⅱ level was negatively related with the severity of depression and anxiety (HAMD:r =-0.5468,P < 0.01;HAMA:r =-0.4146,P =0.001) in PD patients.Conclusions The serum ANG Ⅱ level which is significantly associated with the severity of depression and anxiety in PD patients might become a new clinical biomarker in predicting and diagnosing depression and anxiety in PD patients.
