Objective To study the cognitive impairment (CI) and its influencing factors in ≥70 years old people in Jianghan Oil Field.Methods A total of 10116 ≥70 years old people in Jianghan Oil Field were included in this study.Their serum levels of blood lipids,blood glucose and Hcy were measured,their hepatic and renal function were tested,their blood viscosity was assayed.Their demographic data were recorded and their CI was detected.The diagnosis of dementia and CI and their influencing factors were analyzed.Results The prevalence of dementia was 44.2％ and that of CI was 35.7％ in the 10116 ≥70 years old people.The prevalence of mild CI,Alzheimer's disease,vascular CI and other types of dementia accounted for 21.7％,7.2％,5.9％ and 0.9 ％ respectively.Multivariate logistic regression analysis showed that female gender,hearing and visual acuity loss,poor sleep quality,diabetes,hypothyroidism,cerebrovascular disease,uremia,hyperhomocysteinemia,and whole blood viscosity were the risk factors for CI while physical exercise,drinking tea,doing housework and cheerful personality were the protective factors for CI.Conclusion CI is easy to occur in old people,especially in old females,suffering from chronic diseases,severe hearing and visual acuity loss,poor sleep quality,preventive measures should thus be taken for CI in such people.%目的 探讨江汉油田社区自然人群中年龄≥70岁老年人认知功能障碍现况及影响因素.方法 采用普查方法,对江汉油田年龄≥70岁的10 116例老年人进行体检,同时用自设问卷调查人口学特征,用痴呆自评8项问卷(AD8)进行认知功能障碍筛查,由专科医师对筛查阳性人员进行痴呆与认知障碍的分类诊断和影响因素分析.结果 10 116例老年人中,AD8筛查阳性率为44.2％.认知功能障碍患病率为35.7％,其中轻度认知功能障碍、阿尔茨海默病、血管性认知障碍、其他类型痴呆分别为21.7％、7.2％、5.9％和0.9％.多因素条件logistic回归分析显示,认知功能障碍的危险因素是女性、听力减退、视力减退、睡眠质量差、糖尿病、甲状腺功能减退、脑血管病、尿毒症、同型半胱氨酸异常、高全血黏度;保护因素有健身、饮茶、家务劳动、性格开朗(P＜0.05,P＜0.01).结论 患多种慢病,听力视力减退、睡眠质量差的老年人,尤其女性容易发生认知功能损害,应开展有针对性的预防.
