肠道菌群与大肠癌发病风险的相关性研究

摘要

Objective To study the level of intestinal flora in colorectal cancer, colorectal adenoma and normal population, and to explore the correlation between intestinal flora and colorectal cancer risk. Method 45 patients with colorectal cancer (colorectal cancer group) and 62 patients with colorectal adenoma (colorectal adenoma group) were enrolled in this study from January 2015 to March 2016, and 60 normal subjects were selected as the control group. The levels of flora were determined by polymerase chain reaction (PCR), and the levels of intestinal microflora were compared between the three groups, in order to analyze the occurrence and development of colorectal cancer. Result The levels of Bifidobacteria and Lactobacilli in colorectal cancer group were significantly lower than those in colorectal adenoma group and control group (P < 0.05), and the levels of Escherichia coli, Enterococcus faecalis and Clostridium were significantly higher than those of control group (P < 0.05). The levels of Enterococcus faecalis, Bacteroides, Fusobacterium, Bacteroides fragilis, Bacteroides uniformis, Bacteroides thetaiotaomicron, Bacteroides ovatus, Bacteroides distasonis, Bacteroides vulgatus, Actinomyces pseudonecrophorus, Clostridium botulinum, and clostridium difficile were significantly higher than those of colorectal adenoma group and control group (P < 0.05). Bifidobacterium and Lactobacillus were negatively correlated with colorectal cancer (P < 0.05). Escherichia coli, enterococcus faecalis, bacteroides, fusobacterium, Bacteroides fragilis Bacteroides thetaiontaomicron, Bacteroides vulgatus, Actinomyces necrophorus, clostridium difficile were positively correlated with colorectal cancer (P < 0.05). Bifidobacterium and Lactobacillus were negatively correlated with TNM staging of colorectal cancer (P < 0.05). Escherichia coli, Bacteroides, Fusobacterium and Bacteroides fragilis were positively correlated with TNM staging of colorectal cancer (P<0.05). Conclusion The status of intestinal flora is related to the occurrence and development of colorectal cancer.%目的 研究大肠癌、大肠腺瘤、正常人群中肠道菌群水平,探讨肠道菌群与大肠癌发病风险的相关性.方法 选取2015年1月至2016年3月本院收治的45例大肠癌患者(大肠癌组)和62例大肠腺瘤患者(大肠腺瘤组)为研究对象,并选取60例正常人纳入对照组,收集三组研究对象的粪便并提取DNA,采用荧光定量聚合酶链反应(polymerase chain reaction,PCR)测定菌群数量,并比较三组研究对象肠道菌群水平,分析肠道菌群与大肠癌发生、发展的相关性.结果 大肠癌组患者双歧杆菌、乳酸杆菌水平均明显低于大肠腺瘤组和对照组(P<0.05),且大肠杆菌、屎肠球菌、梭菌属水平均明显高于对照组(P<0.05),粪肠球菌、拟杆菌属、梭杆菌属、脆弱拟杆菌、单形拟杆菌、多形拟杆菌、卵形拟杆菌、吉氏拟杆菌、普通拟杆菌、坏死梭杆菌、肉毒梭菌与艰难梭菌水平均明显高于大肠腺瘤组和对照组(P<0.05).双歧杆菌和乳酸杆菌与大肠癌发病呈负相关(P<0.05),大肠杆菌、粪肠球菌、拟杆菌属、梭杆菌属、梭菌属、脆弱拟杆菌、多形拟杆菌、普通拟杆菌、坏死梭杆菌、艰难梭菌与大肠癌发病呈正相关(P<0.05);双歧杆菌和乳酸杆菌与大肠癌TNM分期呈负相关(P<0.05),大肠杆菌、拟杆菌属、梭杆菌属、脆弱拟杆菌与大肠癌TNM分期呈正相关(P<0.05).结论 肠道菌群状况与大肠癌的发生、发展密切相关.
