肥胖患者腹腔镜Roux-en-Y胃旁路术后贫血发生率及治疗效果

摘要

Objective To analyze the incidence and treatment effects of anemia in obese patients after laparoscopic Roux-en-Y gastric bypass (LRYGB) surgery.Methods A retrospective study was conducted in obese patients underwent LRYGB in our hospital.All their clinical materials during 1 year after surgery were analyzed.The number and incidence of patients with anemia were calculated and the therapeutic effects were examined.Results The incidence of new-onset anemia was 1.45％,2.90％ and 10.14％ respectively in obese patients at month 3,6 and 12 after surgery.Iron deficiency was the main cause of anemia after LRYGB.Iron deficiency anemia was improved by oral iron,combined with intravenous transfusion therapy when necessary.Conclusion The incidence of new-onset iron deficiency anemia in LRYGB increased rapidly after 1 year,and could be effectively alleviated by proper treatment.Postoperative anemia is not a contraindication for bariatric surgery.Regularly blood routine and serum ferritin tests could help detecting mild iron deficiency anemia and thus prevent severe anemia.Endocrinologists participated in postoperative follow-up could efficiently reduce the incidence of severe anemia.%目的 分析肥胖患者腹腔镜Roux-en-Y胃旁路术(LRYGB)术后贫血发生率及治疗效果.方法 回顾性分析我院行LRYGB并完成术后1年随访肥胖患者的随访资料,统计贫血人数及发生率,并分析治疗效果.结果 LRYGB术后3个月、6个月及1年贫血发生率分别为1.45％、2.90％、10.14％.LRYGB术后贫血类型基本为缺铁性贫血.经口服铁剂,必要时联合静脉输血治疗,缺铁性贫血均能有效改善.结论 LRYGB术后1年缺铁性贫血发生率较前迅速上升,经治疗均可有效改善.术后贫血不能作为代谢手术的禁忌,术后定期复查血常规及血清铁蛋白可发现轻度贫血患者,避免中重度贫血发生;内分泌科医生参与术后随访可减少中重度贫血发生.
