一例食道恶性肿瘤患者PICC置管维护及血栓防治的方法

Objective To probe the effect of preventing throm bogenesis and m aintaning catheter unobstructed in sufferers with cancer of hypercoagulable state by m eans of peripherally inserted central catheter(PICC)m aintenance with sim ultaneous anticoagulant therapy.Methods In November2013,1 case of esophageal cancer patient with sim ultaneous supraclavicular lym phatic m etastasis received PICC indwelling and catheter m aintenance,and sim ultaneous anticoagulant therapy was conducted to prevent phlebothrom bosis during the m aintenance peri-od.Results Thepatientstayedinhospitalfor2monthswithPICC catheterinplace.ColorDopplerultrasoundinveinsshowednosignof throm bogenesis.Conclusion For cancer patient in hypercoagulable state,careful PICC maintenance according to strict m aintenance require-m ents,together with anticoagulant therapy and nursing interventions for relevant factors of phlebothrom bosis can prevent venous throm boge-nesis,enhance the safety of PICC ,and lengthen indwelling tim e,m aintain unobstructed catheter so as to achieve the treatm ent goal.%目的：探讨在外周静脉穿刺置入中心静脉导管( PICC )维护时同步抗凝治疗对高凝状态肿瘤患者防止血栓形成、维持导管通畅的效果。方法2013年11月一例食道恶性肿瘤并发右锁骨上淋巴结转移患者接受PICC置管及导管维护，维护期间进行同步抗凝治疗防治静脉血栓。结果患者在住院2个月期间PICC导管在位通畅，静脉彩超显示无血栓形成。结论对高凝状态肿瘤患者进行PICC置管及维护期间严格按照维护要求进行操作，并采取同步抗凝治疗，针对发生静脉血栓的相关因素采取相应的护理干预措施，可以有效防止静脉血栓形成，提高PICC的安全性，延长导管留置时间，维持导管通畅达到治疗目的。
