牙周基础治疗对糖尿病性牙周炎患者GHbA1c及leptin影响的研究

摘要

目的:评估牙周基础治疗对Ⅱ型糖尿病伴慢性牙周炎(DMCP)患者牙周临床指标、糖化血红蛋白A1c(GHbA1c)及血清瘦素(leptin)的影响.方法:选取DMCP患者和单纯慢性牙周炎患者(CP)各40例,分别于牙周基础治疗前和治疗后1、3个月检查其牙龈出血指数(BI)、探诊出血(BOP)、牙周袋深度(PD)、临床附着丧失(CAL)、菌斑指数(PLI),并检测其GHbAlc、leptin等血液生化指标.结果:基线时,BI、PD、PLI、BOP两组相比均无统计学差异(P＜0.05).治疗后1、3个月时,两组的PD、BI、PLI、BOP均较其基线时有显著改善(P＜0.05),其中以DMCP组改善程度最明显,与CP组相比,上述各指标均有统计学差异(P＜0.05),但DMCP组的CAL与基线相比无明显变化(P＞0.05).治疗后两组的GHbA1c、leptin水平均较基线时明显下降(P＜0.05);但整个试验期间,DMCP组的GHbA1c和leptin水平均始终高于CP组(P＜0.05).结论:牙周基础治疗有助于改善Ⅱ型糖尿病伴牙周炎患者的牙周临床参数,并调节其糖代谢.%AIM:To explore the effects of periodontal basic treatment on periodontal clinical parameters,glycated hemoglobin A1c(GHbA1c) and serum leptin in patients with type Ⅱ diabetes and chronic periodontitis(DMCP).METHODS:40 patients with type DMCP and 40 patients with chronic periodontitis (CP) underwent periodontal basic treatment.The bleeding index(BI),bleeding on probing(BOP),periodontal probing depth(PD),clinical attachment loss(CAL),plaque index(PLI),glycated hemoglobin A1c(GHbA1c) and serum leptin concentration were examined at baseline,1 and 3 months after treatment.RESULTS:There was no significant difference in BI,PD,PLI and BOP between the 2 groups at baseline (P ＜0.05).At the 1st and 3rd month after treatment,PD,BI,PLI and BOP in the 2 groups were significantly decreased (P ＜ 0.05),in DMCP group decreased more than in CP group (P ＜ 0.05),CP group did not show significant change of CAL level (P ＞ 0.05).The levels of GHbA1c and leptin were significantly decreased after treatment (P ＜0.05).GHbAlc and leptin of group DMCP was significantly higher than those of CP group in the whole treatment period.CONCLUSION:Periodontal basic treatment can significantly improve periodontal clinical parameters and regulate glucose metabolism in diabetic patients with chronic periodontitis.
