根管治疗术对重度牙周-牙髓联合病变患牙牙周再生效果的影响

AIM:To investigate the effects of endodontic treatment and the selection of the rapeutic schemeon the periodontal regeneration of the severe combined periodontal endodontic lesions. METHODS:15 cases of severe combined periodontal endodontic lesions were divided into 3 groups(n＝5):periodontal regeneration preopera-tive RCT group,periodontal regeneration half -year before RCT group and periodontal regeneration but none RCT group. The probing depth(PD),clinical attachment loss(CAL),tooth mobility(TM),periodontal bone density(BD) andmasticatory function(MF)were measured before,half and 1 year after treatment,respectively. RESULTS:PD, CAL and BD of the preoperative RCT group was significantly improved half and 1 year after surgery(P＜0.05).The postoperative RCT group showed no improvement half year after surgery,but 1 year after surgery was significantly im-proved (P＜0.05). In the none RCT group,the indicators showed no statistically different compared with those before and after treatment(P＞0.05). And all above clinical indicators of the preoperative RCT group and the postoperative RCT group were superior to those of none RCT group 1 year after operation (P＜0.05). CONCLUSION:Endodontic treatment is beneficial for periodontal tissue regeneration of the severe combined periodontal endodontic lesions.%目的:探讨根管治疗术(RCT)对重度牙周-牙髓联合病变患牙牙周再生效果的影响.方法:采用回顾性研究的方法,选取经牙周基础治疗后的重度牙周-牙髓联合病变患者14例共15个患牙,分为3组(n＝5):RCT后3个月行牙周再生术组(术前RCT组)、牙周再生术后半年行RCT组(术后RCT组)、仅牙周再生术未行RCT组(未行RCT组).比较分析3组牙周再生术前、术后半年及1年的探诊深度(PD)、临床附着丧失(CAL)、松动度(TM)、牙周骨密度及咀嚼功能.结果:PD、CAL及骨密度术前RCT组在牙周再生术后半年及1年时较术前均明显改善(P＜0.05);术后RCT组术后半年时较术前无差异,而术后1年时较术前明显改善(P＜0.05);未行RCT组术后半年及1年较术前相比均无统计学差异(P＞0.05);术后1年时,术前RCT组及术后RCT组均显著优于未行 RCT组(P ＜0.05);3组 TM术后半年及1年较术前相比均无统计学差异(P＞0.05).3组咀嚼功能术后1年与术前相比均有显著改善(P＜0.05).结论:对于重度牙周-牙髓联合病变的患牙,牙周再生术前行完善的根管治疗有利于其牙周组织再生.
