Long-term evaluation of periapical healing following endodontic treatment of teeth with apical periodontitis.

The purpose of this investigation was to evaluate long-term healing of teeth with apical periodontitis treated by dental students according to the protocol of UNC School of Dentistry, Department of Endodontics. A total of 49 patients returned for follow up (N=55 teeth). Healing was assessed radiographically using two methods: the Periapical Index (PAI) and a Clinical Impression of Healing (CIH). Overall favorable healing was found in 69% (PAI) and 90.3% (CIH) of teeth. Of the independent variables assessed, only patient history of tobacco use significantly affected healing in the presence of a preoperative periapical lesion (Chi Square=4.27; p=0.038) although clear differences in healing emerged based on patient diabetic status.; Conclusion. Teeth with chronic apical periodontitis treated close to the radiographic apex, with adequate apical enlargement, and with calcium hydroxide as an interappointment, intra-canal medicament had a success rate comparable to outcomes reported in the endodontic literature.
