丝裂霉素C联合粘弹剂在青光眼小梁切除术中的应用

摘要

目的:探讨在青光眼小梁切除术中联合应用丝裂霉素C (mitomycin C,MMC)和粘弹剂预防术后浅前房发生的临床效果.方法:将64例青光眼患者随机分为研究组(n=32,38只眼)和对照组(n=32,37只眼).研究组在小梁切除术中应用0.4mg/mL的MMC棉片,缝合结膜瓣前在巩膜瓣下及前房内注入少量粘弹剂(1％ gluey Healon).对照组仅应用MMC,而不用粘弹剂.结果:研究组术后有6眼发生浅前房(15.8％),对照组术后有17眼发生浅前房(45.9％),两者差异有统计学意义(P＜0.05)；而在远期滤过泡形成及眼压控制方面两组无显著差异.结论:在应用MMC的青光眼小梁切除术中,如联合应用粘弹剂,可以有效预防浅前房的发生而不影响远期效果.%Objective:To study the clinical effects of using mitomycin C(MMC) and gluey Healon in the glaucoma trabeculectomy. Methods:In the study group (32 cases, 38 eyes) . MMC flakes of 0. 4 mg/mL were used in the trabeculectomy, a little of gluey Healon was injected into anterior chamber and under scleral flap before it was sewed. In control group ( 32 cases, 37 eyes), MMC was used, but gluey Healon was not used. Results: In the study group, there were 6 eyes (15. 8%) of shallow anterior chamber after trabeculetomy; while in control group, there were 17 eyes (49. 9%) of shallow anterior chamber (P< 0. 05) . There was no significant difference between the two groups in the intraocular pressure and the formation of filter-bubble in the long term. Conclusions: Using MMC in combination with gluey Healon in trabeculectomy can prevent the formation shallow anterior chamber, but long-term effect is not influenced.
