目的：探讨老年慢性心力衰竭（CHF）患者血清N末端-脑钠肽前体（NT-proBNP）水平变化与临床心功能分级（ NYHA心功能级别）的相关性。方法对188例60岁以上慢性心力衰竭患者入院初期根据临床表现进行NYHA分级，检测血清N末端-脑钠肽前体（ NT-proBNP）水平，同时行超声心动检查记录左室射血分数（ LVEF值）。结果老年慢性心力衰竭患者血清N末端-脑钠肽前体水平随着NYHA心功能级别的升高而升高（ P＜0．05），与LVEF值呈反向相关，与临床心功能分级相关性较好。结论老年心力衰竭患者血清NT-proBNP水平能较好地反映老年失代偿性慢性心力衰竭患者的心功能状态，是老年CHF的诊断和治疗过程中重要的观察指标。%Objective To investigate the correlations between the changes of N-terminal pro-brain natriuretic peptide(NT-proBNP)levels in elderly patients with chronic heart failure (CHF)and clinical cardiac function class ( New York heart association ,NYHA) .Methods One hundred and eighty-eight patients over 60 years old with chro-nic heart failure were graded by NYHA at initial stage .The serum NT-proBNP levels were assayed .Left ventricular e-jection fraction ( LVEF ) was detected by echocardiogram .Results The NT-proBNP levels in elderly patients with CHF showed an increased tendency accompanied by the severity of heart failure (P<0.05)and lower LVEF,and clin-ical heart function class showed good correlation .Conclusion The NT-proBNP levels in elderl patients with CHF which can well reflect cardiac function of elderly patients with decompensated CHF is an important observation index for diagnosis and treatment in elderly patients with CHF .
