三维斑点追踪技术对冠状动脉慢血流患者左心室收缩功能的评价

摘要

目的采用三维斑点追踪技术评估冠状动脉慢血流( CSF )患者的左心室收缩功能。方法选择44例CSF患者,同期选择冠状动脉造影正常的44例作为对照。采用三维斑点追踪技术测量所有患者左心室纵向应变(GLS)、左心室径向应变(GRS)、左心室圆周应变(GCS)及左心室面积应变(GAS)。结果两组GAS、GCS和GRS比较,差异均无统计学意义(均为P>0.05)。与对照组比较,CSF组GLS显著减低(-15.9±1.6比-17.8±2.5,t=2.662,P=0.012)。受试者工作特征曲线分析显示,采用GLS截点值为-16.0预测CSF的灵敏度为76.2%,特异度为71.5%,曲线下面积为0.733( P<0.001)。结论 CSF患者的左心室三维纵向收缩功能显著减低。%Objective To evaluate the longitudinal left ventricular ( LV ) systolic function in the patients with coronary slow flow(CSF)using 3-dimensional speckle-tracking echocardiography(3D-STE). Methods A total of 44 patients with CSF and 44 normal controls were enrolled into this study.The LV global longitudinal strain ( GLS ) , endocardial area strain ( GAS ) , global circumferential strain ( GCS ) and radial strain ( GRS ) were measured using three-dimensional speckle tracking echocardiography. Results GAS, GCS, GRS were not significantly different between two groups ( P >0.05).Compared with control group ,GLS reduced in the CSF group ( -15.9 ±1.6 vs.-17.8 ±2.5,t=2.662,P=0.012).Receiver operating characteristic curve analysis revealed a cut-off point of -16.0% for GLS with a sensitivity of 76.2% and a specificity of 71.5%(area under curve=0.733, P<0.001). Conclusions Longitudinal LV systolic function is significantly reduced in coronary slow flow patients .
