背景:目前的研究表明,骨髓水肿(bone marrow edema, BME)是引起膝骨关节炎(knee osteoarthritis, KOA)疼痛的重要原因,但BME在多大程度上影响KOA疼痛尚存争议.目的:探讨BME与KOA患者疼痛的相关性.方法:2015年2月至2016年6月收治79例伴有BME的老年KOA患者作为观察组,68例不伴有BME的KOA患者作为对照组,对所有患者实施视觉模拟(VAS)评分及应用3.0 T MRI行膝关节平扫,根据BME的半定量分析进行评分,分析BME与KOA疼痛之间的关系.结果:在伴有BME的KOA患者中,BME越明显,疼痛程度越明显,二者呈正相关(P<0.05).结论:BME是导致老年KOA疼痛的重要因素,且KOA患者的疼痛程度与BME严重程度呈正比.%Background: Current studies have shown that bone marrow edema (BME) is an important cause of the pain in knee osteoarthritis (KOA), but the correlation between BME and KOA pain is still controversial. Objective: To explore the correlation between the degree of BME and KOA pain in elderly patients. Methods: From February 2015 to June 2016, 79 KOA patients with BME were assigned to observation group, and 68 KOA patients without BME were divided into con-trol group. Visual analogue scale (VAS) was used to assess the degree of KOA pain. Whole-organ MRI scoring (WORMS) was used for semiquantitative analysis of BME. Results: There is a great interdependency between BME and knee pain (P<0.05). Conclusions: BME is an important factor leading to knee pain in elderly patients, and the degree of pain in the knee is positively related to the severity of BME.
