探析糖尿病肾病与脾的密切关系

摘要

糖尿病肾病的主要表现为蛋白尿、高血压、水肿、肾功能不全等.中医学把本病归属于消渴而继发的“肾劳”“水肿”“关格”“尿浊”等范畴,病变虽位于脾、肺、肾三脏,但脾与糖尿病肾病的发病关系尤为密切.脾失健运为其关键病机,脾虚乃本病之根本并贯穿疾病始末,病理基础由脾及肾.对于糖尿病肾病的治疗,不可拘泥于单纯地从肾论治,而应辨证辨病论治,并注重病变早期,重用健脾之药,确保气血生化有源,补其后天之本以助先天,惟有脾气健运方能滋生万物,固下脱之阴精,化涸竭之津液,以逆转和延缓病程发展.%The main manifestations of diabetic nephropathy are proteinuria,hypertension,edema,and renal dysfunction.The disease belongs to the category of "kidney trouble","edema","Guan Qi" and "urine turbidity" which are secondary to diabetes in TCM.Although the lesion is located in the spleen,lung and kidney three organs,the relationship between the spleen and diabetic nephropathy is especially close.Spleen deficiency in transformation are the key pathogenesis.Spleen deficiency is the root of the disease and runs through the whole course of the disease.The pathological basis is the spleen and kidney.For the treatment of diabetic nephropathy,not only from the kidney on the simple treatment,and syndrome differentiation,disease differentiation,and focus on the early lesions,reuse of spleen medicine,ensure blood biochemical active,make it the day after tomorrow to help birth,only temper health movement,can breed all things,removal of the solid Yin,body fluid of dry,to reverse and slow the progression of the disease.
