改良颊侧多曲簧治疗早期骨性Ⅲ类错牙合软硬组织变化研究

摘要

目的:探讨应用改良颊侧多曲簧活动矫治器矫治早期骨性Ⅲ类错牙合患者,比较矫治前后软硬组织测量值的变化.方法:早期骨性Ⅲ类错牙合患者30名,采用改良颊侧多曲簧活动矫治器,矫治前后拍摄X线头颅侧位定位片并制取模型,测量软硬组织参数,采用SPSS16.0软件进行统计学分析,比较矫治前后软硬组织变化情况.结果:改良颊侧多曲簧活动矫治器矫治早期骨性Ⅲ类错牙合,治疗前后头影测量的SNA(°)、ANB(°)、Y-axis(°)、Ns-Sn-Pos(°)、UL-EP(mm)、LL-EP(mm)项目比较均有显著性差异(P<0.01),上颌尖牙和第一前磨牙区牙弓宽度、第一前磨牙区基骨宽度前、后测量项目的比较均有显著性差异(P<0.01),上颌牙弓长度、上颌基骨长度、下颌第一前磨牙区牙弓宽度和下颌基骨的宽度前、后测量项目比较均有差异(P<0.05).结论:应用改良颊侧多曲簧活动矫治器治疗早期骨性Ⅲ类错牙合患者,能够促进上颌骨生长,增加上颌凸度和宽度,限制下颌骨的过度生长,显著改善侧貌.%Objective To study the variations of soft and hard tissue profile parameters before and after early skeletal Class Ⅲ malocclusion treatments using improved Buccal Multiloop removable appliance. Methods 30 patients with early skeletal class Ⅲ malocclusion were treated with improved Buccal Multiloop removable appliance. Cephalometrics and teeth modeling were performed before and after the treatment, by which the parameters of soft and hard tissue were measured. A statistical analysis was carried out with SPSS V16.0 to compare the changes of soft and hard tissue. Results: After the treatments of early skeletal class Ⅲ malocclusion, the values of SNA(°), ANB(°), U1-NA(°), Y-axis(°),Ns-Sn-Pos(°), UL-EP(mm) and LL-EP(mm) are significantly differently from those measured before the treatments(P<0.01). The dental arch widths of cuspid teeth, the first premolars maxillary, and the first premolars maxillary basal bone are all obviously significantly different(P<0.01). The lengths of the upper dental arch and basal bone, and the widths of the lower first premolars dental arch and basal bone are significantly different from those before the treatment (P<0.05). Conclusion In the treatments of early skeletal class Ⅲ malocclusion, the utilization of improved Buccal Multiloop removable appliance can promote the growth of maxillary, increase its convexity and width, limit the excessive growth of mandible and improve the profile.
