孤立性肺结节容积倍增时间在鉴别良恶性结节中的应用价值

摘要

目的:探讨孤立性肺结节(solitary pulmonary nodules,SPN)容积倍增时间在鉴别良恶性结节中的价值. 方法:收集至少行2次胸部CT检查并有完整病理资料的20例SPN患者,获得结节的生长速度或容积倍增时间,对良恶性结节的容积倍增时间进行比较.结果:20例中,13例结节体积增大,3例体积不变,4例体积缩小.13例结节体积增大者,12例为恶性结节,1例良性结节;3例体积不变和4例结节体积缩小结节均为良性结节. 恶性结节的生长速度较良性快,差异有统计学意义(P<0.05). 容积倍增时间与患者的年龄、性别、复查时间间隔、SPN初始体积等因素均无统计学相关性,容积倍增时间为独立性因素. 结论:容积倍增时间对评估SPN良恶性有良好的临床应用价值.%Objective:Utility of measuring doubling time(DT) in the evaluation of solitary pulmonary nodule. Methods:20 pa-tients with pathologically and clinically proved SPN which undergo at least two chest CT examinations were collected to mea-sure the growing speed or doubling time of the lesion. Results:In 20 cases which had regular visits,SPN volume increased in 13 cases,shrinked in 4 cases,no changing in 3 cases,12 cases with volume increasing were verified malignant tubercle by pathology,others were benign nodules. The growing speed of malignant tubercle was faster than benign nodules. There was no correlation of SPN volume DT to patient age、gender、interval time and first-volume. Volume DT was independent factor. Con-clusion:Doubling time has the great value on the diagnosis of SPN by distinguishing the lung tubercle effectively.
