急性胰腺炎累及右侧肾上腺CT表现及其与胰腺外炎症评分的相关性

摘要

Objective:To explore CT findings of acute pancreatitis involving the right adrenal gland (RAG) and the correlation with extra-pancreatic inflammation on abdominal computed tomography (EPIC) score. Methods:95 cases with acute pancreatitis, which were proved by a typical clinical presentation with elevated serum amylase levels and/or pathologic findings,were includ-ed in this study. Emphasis was placed on the following:manifestations of RAG and the EPIC score of all the cases. Results:①The EPIC score of 36 patients was 0~3,the EPIC score of 59 patients was 4~7. The RAG involvement was detected in 16 patients. ②The involvement of RAG showed a statistically significant association with the EPIC score of the patients with AP (r=0.3587,P<0.05). Conclusion:CT can demonstrate the details of RAG involvement in acute pancreatitis,which can reflect the severity of the acute pancreatitis in a certain extent.%目的：探讨急性胰腺炎（acute pancreatitis，AP）累及右侧肾上腺（right adrenal gland，RAG）的CT表现及其与胰腺外炎症（extrapancreatic inflammation on abdominal computed tomography，EPIC）评分的相关性。方法：回顾性分析95例行全腹CT扫描AP患者的资料，着重观察RAG改变，并计算出EPIC评分。结果：①EPIC评分0～3分36例，4～7分59例；②RAG受累16例，表现为局部边缘与胰周积液分界不清，可合并RAG密度降低、形态异常等；③AP累及RAG的CT表现与其EPIC评分有相关性（r=0.3587，P<0.05）。结论：CT可充分显示AP累及RAG的情况；RAG的受累情况与AP的严重程度呈正相关。
