2型糖尿病合并下肢血管病变患者与晚期糖基化终末产物及脂联素相关性研究

摘要

目的 探讨2型糖尿病及其合并下肢血管病变患者血清晚期糖基化终末产物(AGEs)及脂联素(ADPN)水平的变化.方法 用酶联免疫吸附试验法测定76例2型糖尿病合并下肢血管病变患者(A组),68例单纯2型糖尿病患者(B组)及59例健康者(C组)的血清晚期AGEs及ADPN水平.结果 3组受检者体质指数(BMI)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)水平比较,差异均无统计学意义(P>0.05);而空腹血糖(FPG)、糖化血红蛋白(HbA1c)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、AGEs、ADPN水平比较,差异均有统计学意义(P<0.05).其中与C组相比,A组及B组ADPN水平均明显降低(P<0.05),而晚期AGEs、FPG、HbA1c、TG、LDL-C均明显升高(P<0.05);与B组相比,A组晚期AGEs水平升高(P<0.05),A组ADPN水平降低(P<0.05).AGEs与FPG、LDL-C、HbA1c均呈正相关(P<0.05);ADPN与BMI、FPG均呈负相关(P<0.05);ADPN与HDL-C呈正相关(P<0.05).结论 晚期AGEs、ADPN与糖尿病及下肢血管病变的发生及发展相关.晚期AGEs、ADPN可作为早期诊断糖尿病下肢血管病变及预测其预后的参考指标.%Objective To explore the relationship between advanced glyc:ation end products ( AGEs ) and adiponectin ( ADPN ) in patients with type diabetes mellitus ( T2DM ) accompanied with peripheral vascular disease ( PVD ). Methods The levels of AGEs and adiponectin were determined hy enzyme linked immunosorbent assay ( ELISA ) in 76 patients with T2DM with PVD ( group A ), 68 patients with T2DM only ( group B ), and 59 healthy subjects ( group C ) . Results Body mass index ( BMI), total cholesterol ( TC ). and HDL - C were not significantly different among three groups ( P > 0. 05 ), while FPG, HbA1c , TG, LDL - C AGEs, and ADPN showed significant differences ( P <0. 05 ) . In which ADPN decreased significantly in hoth A and B groups compared with C group, and AGEs, FPG, HbA1c , TG, and LDL - C significantly increased ( P< 0. 05 ) . AGEs increased and ADPN decreased in A group compared with B group ( P < 0. 05 ) . AGEs were positively correlated with FPG, LDL - C, and HbA1c ( P <0. 05 ) . ADPN was negatively correlated with BMI and FPG, and positively correlated HDL - C ( P <0. 05 ) . Conclusion AGEs and ADPN are related with the occurrence and development of T2DM accompanied with PVD. AGEs and ADPN may he used as indicators for the early diagnosis and prognosis of the condition.
