肝部分切除术不预防性放置腹腔引流的护理体会

摘要

目的：探讨肝部分切除术不预防性放置腹腔引流管的围手术期护理策略。方法：回顾性分析2011年1月-2012年10月在笔者所在医院肝脏外科实施肝脏部分切除术后不预防性放置腹腔引流患者275例资料，总结分析围手术期护理体会。结果：无围手术期死亡病例发生。术后外科并发症共12例，其中腹腔出血1例，胆漏1例，膈下积液2例，胸腔积液4例，腹腔感染2例，切开感染裂开2例。患者术后腹部疼痛症状明显减轻，恢复较快。结论：不预防性放置腹腔引流管的肝部分切除术围手术期应密切观察病情变化，精心细致的护理，促进术后的康复，减少术后并发症发生。%Objective:To investigate the partial hepatectomy without preventive intra-abdominal drainage of perioperative nursing strategy. Method:A retrospective analysis of 275 surgical patients receive partial hepatectomy without preventive intra-abdominal drainage form January 2011 to October 2012 in hepatobiliary surgery of our hospital,summarize nursing experience of postoperative complications.Result:No perioperative deaths occurred.A total of 12 cases of surgical complications,including abdominal bleeding in 1 case,fistula in 1 case,under the diaphragm effusion in 2 cases,pleural effusion in 4 cases, abdominal infection in 2 cases,split incision infection in 2 cases.Patients postoperative significantly reduce postoperative abdominal pain symptoms,and recover faster.Conclusion:Postoperative observation patient's condition should closely,careful and meticulous care after the partial hepatectomy without preventive intra-abdominal drainage.To promote postoperative recovery,reduce the postoperative complications and improve the security of liver resection.
