手术治疗重型胰腺炎50例疗效观察

摘要

目的：观察手术治疗重型胰腺炎的疗效。方法：对50例重型胰腺炎患者予以手术切开胰腺被膜，对坏死范围较为明显者给予彻底清除坏死组织，对坏死范围不明或病情危重者仅行部分切除，并仔细探查胆管是否有梗阻，同时给予处理。结果：本组50例患者治愈42例，死亡8例。在治愈的42例中，有5例形成胰腺脓肿或假性囊肿，经再次手术引流而愈，有2例形成胰瘘，经保守治疗1个月痊愈。结论：急性重症胰腺炎时胰腺自身消化的局部病变，采取早期手术，消除坏死组织可以阻止病变的发展。%Objective:To observe the therapeutic effect of operation in the treatment of severe pancreatitis.Method:50 patients with severe pancreatitis operation incision to pancreatic necrosis range,the more obvious to the complete removal of necrotic tissue,necrosis of unknown or critical condition only partial resection,and to explore whether there was obstruction of bile duct,and give treatment.Result:Of the 50 cases 42 were cured,8 cases died.In the cure of 42 cases,5 cases of pancreatic abscess or pseudocyst formation after operation,drainage and more,there were 2 cases of pancreatic fistula,1 month recovered after conservative treatment.Conclusion:Local lesions in severe acute pancreatitis pancreatic autodigestion,and the early operation can eliminate the development of necrotic tissue.
