癌症患者同期放化疗致Ⅳ度骨髓抑制的护理

摘要

目的：探讨实行管床责任护士与患者一对一，加强对同期放化疗致Ⅳ度骨髓抑制患者护理干预的作用。方法：对2011年1月-2013年6月在笔者所在科行放化疗致Ⅳ度骨髓抑制51例恶性肿瘤患者，随机分为试验组和对照组，试验组26例，对照组25例，对照组给予常规护理，试验组除常规护理外，在Ⅳ度骨髓抑制期间指定一名管床责任护士全程给予心理干预，配合饮食护理，给患者家属提供有效的指导等全面、系统的综合护理干预。结果：干预后两组患者出现肺部感染发生率比较差异有统计学意义(P<0.05)，两组WBC>10.00×109/L的平均时间(d)比较差异有统计学意义(P<0.05)，出现口腔溃疡发生率比较差异有统计学意义(P<0.05)。结论：对骨髓抑制患者实行管床责任护士与患者一对一，给予系统、全面的护理及给予家属提供有效的指导能尽快促进患者的恢复，减少并发症的发生。%Objective:To discuss the effect of fixing responsibility nurse and patients with one to one,to strengthen the effect of nursing intervention on cancer patients with Ⅳdegrees of bone marrow suppression caused by concurrent radiochemotherapy.Method:51 cases of malignant tumor patients of Ⅳdegrees of bone marrow suppression caused by concurrent radiochemotherapy from January 2011 to June 2013 in author’s department,they were randomly divided into experimental group and control group,experimental group was 26 cases,control group was 25 cases,the control group was given conventional nursing, the experimental group in addition to routine care,during the period ofⅣdegree bone marrow suppression specified a fixed duty nurse intervention process, cooperated with diet nursing,and given the comprehensive nursing intervention for family members of patients.Result:After the intervention the rate of lung infection in the two groups was significant difference(P<0.05),the average time(d) of WBC above 10×109/L in two groups showed significant differences(P<0.05), the rate of oral ulcer in two groups was significant differences(P<0.05).Conclusion:For the patients withⅣdegrees of bone marrow suppression,the nursing of fixing duty nurse and giving system,comprehensive care and effective direction for their families,can promote the recovery of patients,reduce the incidence of complications.
