真菌性鼻窦炎26例临床与CT诊断分析

摘要

目的：回顾性总结真菌性鼻窦炎CT影像表现，并结合临床，探讨CT检查对诊断真菌性鼻窦炎的临床价值。方法：26例患者均采用骨窗和软组织窗鼻窦CT扫描，确诊为真菌性鼻窦炎，根据患者的CT检查结果并结合临床资料，进行分析比较。结果：病变大部分只累及一侧上颌窦16例，小部分同时累及同侧鼻腔并伴有息肉7例，累及双侧上颌窦1例，筛窦、蝶窦单独发病各1例。病理检查均见菌丝、孢子。CT表现：病灶内斑点、条片样钙化23例，窦壁骨质增厚17例，窦腔缩小13例，骨壁破坏3例。结论：真菌性鼻窦炎具有较为典型的CT表现，并可清晰显示病变累及的范围，对临床的诊断和手术治疗有重要意义。%Objective:To summarize CT features images of fungal sinusitis,and combined with clinical,to explore the clinical value of CT scan in the diagnosis of fungal sinusitis.Method:26 patients with bone and soft tissue window sinus CT scanning,confirmed the diagnosis of fungal sinusitis,according to CT examination of the patient’s results combined with clinical data,analysis and comparison.Result:The lesions mostly involved maxillary sinus in 16 cases,a small part in the same side of the nasal cavity and accompanied by polyps in 7 cases,1 cases of bilateral maxillary sinus,ethmoid sinus,sphenoid sinus disease with 1 cases in each individual.CT features:Intralesional dot strip calcification in 23 cases,sinus wall bone thickening in 17 cases,sinus cavity was reduced in 13 cases,bone destruction in 3 cases.Conclusion:The CT findings of fungal sinusitis is typical,and can clearly display the anatomic extent,is important for diagnosis and treatment of clinical operation.
