口腔正畸固定矫治器应用中牙釉质脱矿的临床探讨

摘要

Objective:To analyze the enamel demineralization incidence and degree of injury in the application process of fixed orthodontic appliance. Method:41 patients with fixed orthodontic appliances for correction were randomly selected in the dental clinic from May 2012 to May 2013 as the observation objects,and the clinical data were retrospectively analyzed in orthodontic treatment,after the end of the overall position of tooth enamel demineralization was summarized,and the enamel demineralization index (EDI) was calculated.Result:24 patients with fixed orthodontic appliance appeared enamel demineralization, the incidence rate was 58.5%.In 946 teeth,112 teeth enamel demineralization occurred,the incidence rate was 11.8%.The enamel enamel incidence of maxillary teeth was 73.2%,which was significantly higher than 26.8%of mandibular teeth,the difference was statistically significant(P<0.05).The average of maxillary tooth enamel demineralization index was significantly higher than that of mandibular teeth,the difference was statistically significant(P<0.05).The demineralization degree was level 1,compared with other degrees,the differences were statistically significant(P<0.05).Conclusion:During the process of the application of fixed orthodontic appliance,the incidence of enamel demineralization is higher,so it should be especially paid attention to the maxillary teeth enamel demineralization treatment and prevention.%目的：分析口腔正畸固定矫治器应用过程中牙釉质脱矿发病率及损伤程度。方法：随机抽取笔者所在牙科诊所2012年5月-2013年5月采用正畸固定矫治器进行矫正的41例患者作为观察对象，并对其临床资料进行回顾性分析，对矫正治疗结束后总体牙位牙釉质脱矿情况进行总结，并计算牙釉质脱矿指数(EDI)。结果：正畸固定矫治器应用过程中共有24例患者出现牙釉质脱矿，发病率为58.5%。在946颗牙齿中，有112颗牙齿发生釉质脱矿，发病率为11.8%。其中上颌牙的牙釉质脱矿发病率73.2%明显高于下颌牙的26.8%，差异有统计学意义(P<0.05)；上颌牙的平均牙釉质脱矿指数明显高于下颌牙，差异有统计学意义(P<0.05)；脱矿程度1级患牙居多，与0级、2级和3级比较，差异均有统计学意义(P<0.05)。结论：口腔正畸固定矫治器应用过程中，牙釉质脱矿发生率较高，尤其应该注意对上颌牙牙釉质脱矿进行预防和治疗。
