彩色多普勒测定大脑中动脉血流变化在新生儿缺氧缺血性脑病中的意义

         

摘要

目的:探讨以彩色多普勒评价大脑中动脉血流变化对预测新生儿缺氧缺血性脑病的临床意义。方法:以2014年3月-2016年3月于笔者所在医院分娩的42例有缺氧缺血性脑病症状和体征的新生儿为试验组,另择取同期正常出生的42例健康新生儿为对照组。以东芝-790A 彩超机对两组新生儿的大脑中动脉血流变化和脑组织结构变化进行扫查,对比分析两组间差异。结果:观察试验组新生儿脑组织声像图发现,回声普遍有所增强,程度不一。经彩超测定,试验组患儿 Vm、Vs、Vd 均不同程度的低于对照组(P<0.05),S/D 和 RI 数值普遍高于对照组,差异均有统计学意义(P<0.05)。结论:以彩色多普勒测定大脑中动脉血流变化有利于新生儿缺氧缺血性脑病的早期诊断和治疗,应用安全有效,可靠性高,值得临床推广使用。%Objective:To explore the effect of the changes of cerebral blood flow by color doppler evaluation of clinical significance for predicting neonatal hypoxic ischemic encephalopathy.Method:42 newborns with hypoxic ischemic encephalopathy symptoms and signs in our hospital from March 2014 to March 2016 were selected as the experimental group,the other 42 healthy newborns were selected normal birth control group.The changes of cerebral artery blood flow and the structural changes of the brain tissue of the two groups were scanned by Toshiba -790A color doppler ultrasound machine,the differences between the two groups were compared and analyzed.Result:Observation of neonatal brain tissue in the experimental group found that echo was generally enhanced,the degree was different.The color doppler ultrasound measurement,the experimental group of children with Vs,Vd,Vm were lower than the control group(P<0.05),S/D and RI values were generally higher than the control group,the differences were statistically significant(P<0.05).Conclusion:The application of color doppler in the diagnosis and treatment of cerebral artery blood flow changes is beneficial to the early diagnosis and treatment of hypoxic ischemic encephalopathy,it is safe and effective,and has high reliability,it is worthy of clinical application.
