和肽素联合NT-proBNP对慢性心力衰竭患者严重程度及近期预后的相关性分析

Objective To observe correlation between severity and short-term prognosis by copeptin combined with N-terminal pro-brain natriuretic peptide (NT-proBNP) for chronic heart failure (CHF) patients. Methods There were 80 CHF patients as CNF group and 40 healthy people as control group. Both groups received copeptin and NT-proBNP detection, and their differences in copeptin and NT-proBNP levels were compared, along with level changes in different NYHA cardiac functional classification. The patients were further divided by follow-up outcome into event group and non-event group for comparison of the two indexes. Results CHF group had obviously higher copeptin and NT-proBNP levels as (4497.50±1564.31) and (6970.10±1054.33) pg/ml than (1306.42±94.72) and (124.06±33.55) pg/ml in the control group (P<0.05). Higher cardiac functional classification led to much higher copeptin and NT-proBNP levels in patients (P<0.05). The event group had much higher copeptin and NT-proBNP levels as (4579.82±1153.43) and (8143.66±2202.17) pg/ml than (2054.73±984.65) and (944.83±157.26) pg/ml in the non-event group (P<0.05).Conclusion Combined detection of copeptin and NT-proBNP is helpful for early prediction of severity and prognosis of CHF patients, and this method contains important significance for diagnosis and treatment of disease.%目的：观察和肽素联合N末端脑钠肽前体(NT-proBNP)对慢性心力衰竭(CHF)患者严重程度及近期预后的相关性分析。方法80例CHF患者作为CHF组,40例健康体检者作为对照组,两组均行和肽素以及NT-proBNP检查,比较CHF组与对照组和肽素以及NT-proBNP的水平差别,不同NHYA心功能分级患者两指标的变化,根据患者随访结果再次分为事件组和非事件组,并进行组间两指标的比较。结果 CHF组患者和肽素与NT-proBNP水平(4497.50±1564.31)、(6970.10±1054.33)pg/ml明显高于对照组(1306.42±94.72)、(124.06±33.55)pg/ml(P<0.05)；随着心功能分级的升高,患者和肽素水平和NT-proBNP水平也明显升高(P<0.05)；事件组患者和肽素水平和NT-proBNP水平(4579.82±1153.43)、(8143.66±2202.17)pg/ml明显高于非事件组(2054.73±984.65)、(944.83±157.26)pg/ml(P<0.05)。结论和肽素联合NT-proBNP检测有助于临床医生对CHF患者的病情严重程度以及预后情况做出早期预测,对于疾病诊断以及治疗具有重要意义。
