临床药师参与1例化脓性脑膜炎合并深静脉血栓患者的药学监护

摘要

OBJECTIVE: To discuss the mode and method of clinical pharmacists providing pharmaceutical care in clinical practice. METHODS: Clinical pharmacists participating in pharmaceutical care for a patient with purulent meningitis complicating with deep vein thrombosis (DVT) was analyzed retrospectively. Clincial pharmacists assisted physicians to develop therapeutic regimen. RESULTS & CONCLUSIONS: Through drug selection for anti-infection and anticoagulant and supportive treatment, pharmaceutical care and patient education, clinical pharmacists help physician to provide reasonable therapy regimen to reduce meningitis infection and high clotting significantly and avoid adverse drug reaction so as to improve therapeutic efficacy and life quality of the patient.%目的:探讨临床药师在临床实践中实施药学监护的模式与方法.方法:分析临床药师参与1例化脓性脑膜炎合并深静脉血栓患者的全程药学监护,协助医师制订治疗方案.结果与结论:临床药师从抗感染、抗凝和支持治疗的用药选择、药学监护和患者教育方面,积极配合医师,为患者提供了合理的用药方案,使患者脑膜炎感染和高凝血症状得以明显改善,并避免了严重不良反应的发生,提高了药物治疗效果和患者生活质量.
