小切口单侧半椎板切除入路治疗脊髓肿瘤的效果及安全性分析

摘要

目的：探讨小切口单侧半椎板切除入路治疗脊髓肿瘤的临床效果。方法选择2008年5月~2012年5月于本院就诊的44例脊髓肿瘤患者,简单随机分为试验组(22例)和对照组(22例),试验组采用小切口单侧半椎板切除入路进行手术治疗,对照组采用传统的大切口全椎板切除入路进行手术治疗,观察两组的手术一般情况及治疗后神经功能恢复情况,比较两组的并发症发生率。结果治疗后试验组患者神经功能恢复为Ⅰ、Ⅱ级的比例显著高于对照组,神经功能恢复为Ⅲ、Ⅳ级的比例显著低于对照组,差异有统计学意义(P<0.05)；试验组术中出血量少于对照组,下床时间、住院时间短于对照组,差异有统计学意义(P<0.05)；试验组治疗后并发症发生率为13.6%,低于对照组的40.9%,差异有统计学意义(χ2=4.1250,P=0.0423)。结论小切口单侧半椎板切除入路治疗脊髓肿瘤效果显著,创伤较小,并发症少,安全性高,适合临床长期推广应用。%Objective To analyze the efficacy and safety of unilateral laminectomy small incision approach in the treat-ment of spinal cord tumor. Methods 44 cases of patients with spinal cord tumor treated in our hospital from May 2008 to May 2012 were selected and simple randomly divided into experimental group (22 cases) and control group (22 cas-es).The experimental group were treated with unilateral laminectomy small incision approach,while the control group were treated with traditional large incision laminectomy approach for surgery.The generally surgical situation and cir-cumstances of the recovery of neurological function after treatment,and the incidence of complications of two groups were observed. Results After treatment,the restored nerve function with grade Ⅰ,II level in patients of the experimen-tal group were higher than those in the control group (P<0.05);Ⅲ,Ⅳ grade were lower than those of the control group (P<0.05);the blood loss of experimental group was less than that of the control group,bed time and hospitalization were shorter than those in the control group,the differences were significant (P<0.05).The complication rate of the experimen-tal group was 13.6%,lower than 40.9% of the control group,the difference was significant (P<0.05). Conclusion The ef-ficacy of unilateral laminectomy small incision approach in treatment of spinal cord tumor is significant,has less trauma, fewer complications,the safety is high,it is worthy of clinical application for long-term.
