中医院医院感染横断面调查分析

摘要

Objective To understand the nosocomial infection rate and current management situation in a comprehen-sive traditional Chinese medicine (TCM) hospital as well as the characteristics of hospital infection. Methods By clini-cal rounds and looking up the illness records,cross-sectional investigation and analysis of nosocomial infection in 6 time points from 2012 to 2013 in one comprehensive hospital. Results There were 6773 patients investigated in these 6 time points and 362 participants in 382 cases occurred with hospital infection.The current infection rate in these 6 time points was 4.9%, 5.9%, 5.2%, 6.3%, 4.7%, and 4.9% respectively,at an average of 5.3%.The current infection rate based on ill cases was 5.3%,5.9%,5.7%,6.7%,5.2%,and 5.0% in turn,at an average of 5.6%.With regard to distribution of infection site,upper respiratory tract infection took the first place,and then urinary system infection and lower respira-tory infection accounting for 31.9%,29.1%,and 25.9% among all these medical cases.Among 382 nosocomial infection cases,the hospital infection time was 1 to 39 d,at an average of (20.2±2.3)d. Conclusion Compared with the same level of western medicine hospital,the course of nosocomial infection cases in the TCM hospital is longer and the current in-fection rate is higher.Clinical physicians’ recognition on control and prevention of nosocomial infection is insufficient, which should be paid great attention to management.%目的：了解某综合性中医院医院感染现患率及管理现状，探讨中医院医院感染的特点。方法采用临床查房和查阅病历相结合，分别对某综合性中医院2012~2013年6个不同时间点进行医院感染横断面调查分析。结果6个时间点共调查患者6773例，发生医院感染362例、382次，6个时间点现患率分别为4.9%，5.9%，5.2%，6.3%，4.7%，4.9%，平均现患率5.3%，例次率分别为5.3%，5.9%，5.7%，6.7%，5.2%，5.0%，平均5.6%；感染部位以上呼吸道感染最多，其次为泌尿系和下呼吸道感染，分别占全部例次的31.9%，29.1%和25.9%；382例次医院感染病例，罹患医院感染时间为1~39 d，平均(20.2±2.3)d。结论与同级西医院比较，该中医院的医院感染病例病程较长，现患率偏高，临床医生对医院感染防控的认识不足，在管理时应对此予以重视。
