Objective To discuss clinical therapeutic effects on respiratory failure of noninvasive positive pressure ven-tilation in patients with chronic obstructive pulmonary disease (COPD) acute exacerbation with severe type II. Meth-ods Selected 80 patients with respiratory failure in our hospital from January 2013 to January 2014 treated by COPD exacerbations with severe II type as the research object, according to the random table method they were divided into two groups, the observation group and control group, control group were given the conventional treatment, while the observation group were given on this basis noninvasive positive pressure ventilation, and before and after treatment ar-terial blood gas index changes, antibiotic use time and hospital stay of the two groups were closely observed. Results After treatment, improving PaO2, pH and PaCO2 decline of the observation group was better than control group, the difference was statistically significant (P<0.05). Length of hospital stay, antibiotic use time of observation group was obviously less than control group,the difference was statistically significant (P< 0.05). Conclusion Chronic obstruc-tive pulmonary disease in acute aggravating with severe type II respiratory failure on the basis of conventional treat-ment to practice the noninvasive positive pressure ventilation therapy, treatment effect is remarkable, and can shorten the length of hospital stay and antibiotics use time,is worth promoting and using in clinic.%目的：对无创正压通气治疗慢性阻塞性肺病急性加重合并重度Ⅱ型呼吸衰竭的临床治疗效果进行探讨。方法选取我院2013年1月~2014年1月所收治的COPD急性加重合并重度Ⅱ型呼吸衰竭患者80例作为研究对象，按随机表法将其分成两组，即观察组与对照组。对照组患者实行常规治疗，而观察组患者则在此基础之上实施无创正压通气，密切观察对两组患者治疗前后的动脉血气指标变化情况、抗生素使用时间以及住院时间等。结果治疗后，观察组患者在PaO2、pH改善与PaCO2下降方面明显优于对照组，差异有统计学意义(P<0.05)。观察组的住院时间、抗生素使用时间明显少于对照组，差异有统计学意义(P<0.05)。结论对慢性阻塞性肺病急性加重合并重度Ⅱ型呼吸衰竭在常规治疗的基础上再实行无创正压通气治疗，治疗效果显著，且可缩短住院时间与抗生素使用时间，值得在临床上大力推广应用。
