早吸吮对剖宫产术后初乳分泌时间及泌乳量的影响

摘要

目的 研究早吸吮对剖宫产术后初乳分泌时间及泌乳量的影响. 方法 选取2013年1月~2014年1月我院行剖宫产产妇的单胎足月新生儿100例,根据随机数字法分为观察组(进行早吸吮)和对照组(不进行早吸吮),每组各50例,观察并比较两组产妇产后泌乳始动时间、24 h内哺乳次数、产妇产后1~5 d泌乳量以及两组产妇术前、术后24 h、48 h的PRL水平的变化情况. 结果 观察组产妇泌乳始动时间快于对照组,观察组24 h内哺乳次数明显多于对照组(P<0.05). 观察组产妇1~5 d内泌乳量充足率达68.0%,显著高于对照组乳量充足率40.0%(P<0.05). 而观察组产妇缺乳率仅6.0%,显著低于对照组的缺乳率26.0%(P<0.05). 观察组术后24 h、48 h的PRL水平分别显著高于对照组(P<0.05). 结论 对剖宫产术后新生儿进行早吸吮,其通过促进乳头神经末稍传导垂体前叶引起催乳素释放,有利于提前促进初乳分泌时间,提高产后初期泌乳量,从而进一步提高母乳喂养的成功率.%Objective To study the effect of early sucking on the secretion time and the amount of lactation after ce-sarean section. Methods From January 2013 to January 2014 in our hospital, 100 cases cesarean section parturients of singleton full-term newborns, according to the random number were divided into observation group (early sucking) and control group (not early sucking), each with 50 cases, the initial time of lactation, within 24 hours of feeding times, postpartum 1-5 d lactation, and the women in two groups before operation, postoperative 24 h, 48 h of PRL levels changes were observed and compared. Results In the observation group of women in the initial time of lactation was faster than that of the control group, 24 hours feeding times was significantly higher than that of the control group (P<0.05). The amount of 1-5 d in the observation group was 68.0%, which was significantly higher than 40.0%of the con-trol group(P<0.05). And the maternal lack of milk rate of observation group was only 6.0%, was significantly lower than the control group of 26.0%(P<0.05). The levels of PRL in the observation group after 24 h, 48 h were significantly higher than those in the control group(P<0.05). Conclusion After the early sucking of newborn infants, it is beneficial to pro-mote the secretion of the first time and increase the success rate of breast feeding.
