社区老年高血压患者分级管理模式的研究

摘要

目的 探讨社区老年高血压患者的管理模式.方法 社区老年高血压患者168例,依据高血压严重程度分级、高血压并发靶器官和/或功能损害、高血压合并其他疾病和/或功能损害及生活自理程度等影响高血压患者生活质量的诸多因素,按病情把患者分为Ⅰ～Ⅳ级,根据每级患者的具体情况制定相应的治疗和护理计划.结果 高血压治疗率达100%,总控制率112例(67.1%).Ⅰ、Ⅱ级患者高血压控制率达112例(87.5%),m级患者高血压控制率达15例(71.7%).Ⅳ级患者高血压控制率达6例(32.5%).结论 社区老年高血压患者分级管理是行之有效的模式,可以作为一种尝试进一步研究和推广.%Objective To explore the management models for community-based elderly patients with hypertension.Methods One hundred and sixty eight elderly patients with hypertension were classified into grades Ⅰ～Ⅳ mainly base on the severity,hypertension target organ and/or function impairment.Results Treatment of hypertension was 100% and total control rate (112 cases) is 67.1%.Control rate of grade Ⅰ/Ⅱ,Ⅲ and Ⅳ was 87.5%,71.7%,32.5% respectively.Conclnsion The management models for Community-baaed elderly patients at different levels are well-established.
