低位直肠癌保肛术后腹腔热灌注联合静脉化疗的疗效观察

摘要

目的 观察低位直肠癌保肛术后应用腹腔低渗热灌注化(HIPEC)联合静脉化疗后对局部复发率的影响.方法 将我院2003年1月至2007年3月89例低位直肠癌保肛术后患者用抽签法分为HIPEC联合全身化疗的试验组44例及行单纯全身化疗的对照组45例,试验组行HIPEC 4次,之后行FOLFOX6化疗方案静脉化疗.对照组则行单纯FOLFOX6化疗方案静脉化疗.随访2年,比较2组局部复发率.结果 2组均未发生严重并发症.试验组失访3例,对照组失访5例.试验组和对照组术后2年复发率分别为12.2%(5/41)和37.5%(15/40),差异有统计学意义(P<0.05).结论 术后早期HIPEC联合静脉化疗能有效地降低低位直肠癌保肛术后局部复发率,是低位直肠癌保肛术后有效的辅助治疗方式.%Objective To evaluate the effects of the hyperthermic peritoneal perfusion chemotherapy (HIPEC)combined with venous chemothempy for low rectal cancer after anal sphincter preserving operation.Methods Eighty-nine cases of low rectal cancer after anal sphincter preserving operation were randomly divided into experimental group(n=44)and control group(n=45).HIPEC was used four times,then the FOLFOX6 chemotherapy plan was carried out monthly for six months in experimental group.The control group was given FOLFOX6 chemothempy plan for six months only.The 2-year local recurrent rate was compared between the two groups.Results There were no severe complications occulted in two groups.The 2-year local recurrent rate in experimental group was 12.2%and was 37.5%in control group.The variation of 2-year local recurrent rote between the two groups achieved statistical significance(P<0.05).Conclusion HIPEC combined with venous chemotherapy can reduce the 2-year local recurrent rate of low rectal cancer after anal sphincter preserving operation.
