微创穿刺术治疗新生儿迟发性维生素K缺乏致颅内血肿临床观察

摘要

Objective To discuss the clinical effect of microtrauma puncture operation for newborns with intracranial hematoma caused by delayed vitamin K deficiency.Methods Eighty-six cases from June.2006 to Septembein.2010 were divided into control group and observation group.Forty-one cases in control group were given conventional treatment,while 45 cases in observation group were treated with microtrauma puncture operation.The brain function impairment and neurological function impairment degree before and after operation as well as hospitalization time and expenses were compared.Results Effective rate of observation group was 93.3%,morbidity was 4.4% ( P < 0.01 ) ; Effective rate of control group was 63.8%,morbidity was 24.4%.Observation group's efficacy was obviously higher than control group's,and disability rate,hospitalization time and expenses were lower than those in control group (P < 0.05).Conclusions Timely operation for hematoma removal after correcting coagulation function can effectively reduce brain function impairment and neurological function impairment.It also can reduce hospitalization time and expenses and decrease fatal and disability rate.%目的 探讨微创穿刺术治疗新生儿迟发性维生素K缺乏致颅内血肿的临床疗效.方法 迟发性维生素K缺乏致颅内血肿新生儿86例根据患儿家属自愿的原则而采取的治疗措施分为对照组和观察组.对照组41例采用内科保守治疗,观察组45例采用微创穿刺术治疗,并比较手术前后脑功能损害和血肿压迫引起的神经功能损害程度以及住院时间和住院费用.结果 观察组的有效率为93.3%,致残率为4.4%,对照组的有效率为63.8%,致残率为24.4%.观察组患儿住院时间少于对照组[(27.89±5.47)d比(69.24±8.35)d,P<0.01],住院费用少于对照组[(4.13±1.16)万元比(6.28±1.63)万元,P<0.05].结论 微创穿刺术在凝血功能纠正后应及时手术清除血肿,可有效减轻新生儿黄疸所致的脑功能损害和血肿压迫引起的神经功能损害,减少住院时间、住院费用,减少致死率和致残率.
